摘  要:目的:探讨低管电压和低浓度造影剂对下肢动脉血管成像质量和辐射剂量的影响。方法:选择2016年2月至2017年2月沈阳医学院附属第二医院行下肢动脉CT血管造影术(CTA)的患者88例为研究对象,随机分为观察组与对照组,各44例。其中观察组患者下肢动脉CTA方案为管电压100 kVp,造影剂为270 mg I/ml的威视派克;对照组患者下肢动脉CTA方案为管电压120 kVp,造影剂为350 mg I/ml的威视派克。对比2组图像评分、图像噪声、下肢动脉CT值、对比噪声比、辐射剂量以及造影剂碘摄入量情况。结果:2组图像评分差异无统计学意义(P>0.05)。观察组下肢动脉CT值、对比噪声比均显著高于对照组(P<0.05),观察组容积剂量指数、辐射剂量长度乘积、有效辐射剂量、碘摄入量均显著低于对照组(P<0.05)。结论:低管电压和低浓度造影剂可为临床提供更有效的下肢动脉血管图像,且有效降低了辐射剂量,具有较好的安全性,值得临床推广应用。Objective:To investigate the effect of the lower tube voltage and low concentration of contrast medium on lower limb artery imaging quality and radiation dose. Methods:A total of 88 cases of patients underwent lower limb artery CT angiography(CTA)from Feb 2016 to Feb 2017 in our hospital were selected and were divided into observation group and control group,each group of 44 cases. In the observation group,the tube voltage was 100 k Vp and the concentration of contrast medium(Visipaque)was270 mg I/ml. In the control group,the tube voltage was 120 k Vp and the concentration of contrast medium(Visipaque)was 350 mg I/ml. Image scores,image noise,CT value,noise ratio,radiation dose and contrast medium iodine intake were compared between the two groups. All images were assessed by two radiological specialist. Results:There was no statistically significant difference in image scores between the two groups(P>0.05). The CT value and noise ratio in the observation group were significantly higher than those in the control group(P<0.05). The CT dose index of volume,radiation dose length product,effective radiation dose and iodine intake in the observation group were significantly lower than those in the control group(P<0.05). Conclusion:Lower tube voltage and lower concentration of contrast medium can provide more effective lower limb artery image,and effectively reduce the radiation dose,with good safety,which is worthy of clinical application.
