升清降浊汤联合健康教育及二甲双胍治疗2型糖尿病(?痰湿?)随机平行对照研究

摘　　要：[目的]观察升清降浊法联合健康教育及二甲双胍治疗2型糖尿病(痰湿)疗效。[方法]使用随机平行对照方法,将108例门诊患者按就诊顺序号抽签方法随机分为两组,健康教育,糖尿病饮食。对照组54例二甲双胍,0.25g/次,3次/d(根据患者情况调整,≤2g/d)。治疗组54例升清降浊汤:黄连6g,葛根15g,黄芪30g,绞股蓝15g,僵蚕10g,蝉蜕6g,姜黄10g,大黄6g,泽泻15g,水煎300ml,2次/d;二甲双胍治疗同对照组。连续治疗15d为1疗程。观测临床症状、糖化血红蛋白(HbA1c)、BMI、抵抗素、IGF-1水平、不良反应。连续治疗2疗程(30d),判定疗效。[结果]治疗组显效35例,有效14例,无效5例,总有效率90.70%;对照组显效18例,有效21例,无效15例,总有效率72.22%;治疗组疗效优于对照组(P<0.01)。Hb A1c、BMI、抵抗素、IGF-1两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。[结论]升清降浊汤联合健康教育及二甲双胍治疗2型糖尿病(痰湿),疗效满意,无严重不良反应,值得推广。[Objective] To observe the curative effect of ascending turbidity and descending turbidity combined with health education and metformin in the treatment of type 2 diabetes mellitus（phlegm dampness）. [Method] Randomized parallel comparative method was used, 108 outpatients were randomly divided into two groups according to the order number, health education, diabetic diet. 54 cases in the control group, metformin, 0.25 g/times, TID（Adjust according to patient＇s condition, ≤2 g/d）. 54 cases in treatment group, ascending the clear and descending the turbid tang： rhizoma coptidis 6 g, the root of kudzu vine15 g, astragalus membranaceus 30 g, gynostemma pentaphylla15 g, silkworm larva10 g, periostracum cicada 6 g, turmeric 10 g, rheum officinale 6 g, rhizoma alismatis15 g, decoct with water 300 ml, bid; metformin treatment was similar to control group. Continuous treatment for 15 days is 1 courses. Observation of clinical symptoms, level of Hb A1 c, BMI, resistin, IGF-1, untoward effect. Judge curative effect after continuous treatment for 2 courses（30 d）. [Result] The treatment group was markedly effective in 35 cases, effective in 14 cases, 5 cases were invalid, the total effective rate was 90.70%; The control group was markedly effective in 18 cases, effective in 21 cases, 15 cases were invalid, the total effective rate was 72.22%; The curative effect of the treatment group is better than that of the control group（P0.01）. Level of Hb A1 c,BMI, resistin,IGF-1 in the two groups were improved（P0.01）. The treatment group was better than the control group（P0.01）.[Conclusion] The curative effect of on ascending the clear and descending the turbid tang on type 2 diabetes mellitus（phlegmatic hygrosis）combid with health education and metformin is satisfactory,and no serious adverse reactions were observed, they seem worth promoting.
