联合应用高频电刀、冷冻、球囊扩张治疗良性气道狭窄的疗效分析

摘　　要：目的 观察电子支气管镜下联合应用高频电刀、冷冻、球囊扩张治疗良性气道狭窄的疗效.方法 选择良性气道狭窄的患者24例,给予支气管镜下联合应用高频电刀、冷冻及球囊扩张治疗.所有患者至少随访12个月,观察患者术后即时、中期（术后3个月）、远期（术后12个月及以上）的疗效.结果 24例患者共进行128次介入治疗,即时、中期（术后3个月）、远期（术后12个月及以上）总有效率均为100％.治疗前呼吸困难指数与治疗后即时、中期、远期对比差异均有统计学意义（P〈0.05）;治疗后即时与中期呼吸困难指数对比差异有统计学意义（P〈0.05）;中期与远期呼吸困难指数对比差异无统计学意义（P〉0.05）.24例患者中,未见大咯血、气胸、纵隔气肿等严重并发症.结论 联合应用高频电刀、冷冻、球囊扩张治疗气道狭窄疗效确切,即时及中远期疗效均良好,并发症少,值得进一步推广应用.Objective To evaluate the effectiveness of bronchoscopy interventional therapy of benign airway stenosis ,and to compare the clinical value of the combined interventional approach being used .Methods 24 patients with benign airway stenosis were treated with joint electrocautery , the bronchial balloon dilatation and cryotherapy .These patients were followed up for at least 12 months and were observed immediate ,the medium-term （3 months later） and long-term （12 months later） efficacy . Results 24 patients were treated with 125 cycles of bronchoscopy interventional therapy .The total effective rate was 100% in the immediate ,medium （ 3 months later） ,long term （ 12 months later） .The dyspnea index of the patients showed significant difference between prior treatment and immediate ,prior treatment and medium ,prior treatment and long-term （ P 〈 0 .05） .There showed significant difference between immediate and medium （ P 〈 0 .05） .There was no significant difference between medium and long-term （P 〈 0 .05） .There were no hemoptysis ,pneumothorax ,mediastinal emphysema and other serious complications in 24 patients .Conclusions Combined electrocautery ,cryotherapy and the bronchial balloon dilatation for benign airway stenosis ,the medium-term and long-term efficacy is good .They have few complications .
