益阴活络汤治疗2型糖尿病周围神经病变45例临床观察

摘　　要：目的:观察益阴活络汤治疗2型糖尿病周围神经病变的临床效果。方法:将90例符合纳入标准的2型糖尿病周围神经病变患者随机分为2组各45例。对照组予常规西药治疗,治疗组在对照组基础上予益阴活络汤治疗,4周为1个疗程,治疗1个疗程后比较疗效。记录2组治疗前后症状(麻木、疼痛、感觉异常)改善情况,测定2组治疗前后空腹及餐后2h血糖(FBG、PBG)、空腹胰岛素(Fins)、糖化血红蛋白(HbA1c),计算胰岛素抵抗指数(HOMA-IR),评定2组综合疗效。结果:治疗后2组手足麻木、疼痛、感觉迟钝症状均较治疗前有改善,总有效率治疗组为91.1%,对照组为75.6%,2组比较,差异有统计学意义(P<0.05);治疗后2组FBG、PBG、Fins、HbA1c、HOMA-IR均较治疗前明显下降(P<0.05),治疗组改善优于对照组(P<0.05)。结论:益阴活络汤能明显改善2型糖尿病周围神经病变患者的胰岛素抵抗功能及肢体麻木、疼痛、感觉迟钝等临床症状,具有较好疗效。Objective：To investigate the clinical effect of Yiyin Huoluo decoction in the treatment of type 2 diabetic peripheral neuropathy.Methods：A total of 90 patients with type 2 diabetic peripheral neuropathy who met the inclusion criteria were enrolled and randomly divided into control group and treatment group,with 45 patients in each group.The patients in the control group were given conventional Western medicine treatment,and those in the treatment group were given Yiyin Huoluo decoction in addition to the conventional Western medicine treatment.Each course of treatment was 4 weeks,and the clinical outcome was compared between the two groups after one course of treatment.The improvement in symptoms（numbness,pain,and paresthesia） was recorded after treatment;fasting blood glucose（FBG）,2-hour postprandial blood glucose（2 h PG）,fasting insulin（FINS）,and hemoglobin A1c（HbA1c） were measured before and after treatment;homeostasis model assessment-estimated insulin resistance（HOMA-IR） was calculated,and the treatment outcome was evaluated for both groups.Results：Both groups had some improvement in the symptoms of numbness,pain,anddysesthesia after treatment and there was a significant difference in overall response rate between the treatment group and the control group（91.1% vs 75.6%,P0.05）.Both groups had significant reductions in FBG,PBG,FINS,HbA1c,and HOMA-IR after treatment（P0.05）,and the treatment group had significantly greater reductions than the control group（P0.05）.Conclusion：Yiyin Huoluo decoction can significantly improve insulin resistance and the symptoms of numbness,pain,anddysesthesia and has a good clinical effect in the treatment of type 2 diabetic peripheral neuropathy.
