摘　　要：目的:观察改良手术方式加中医定向透药疗法治疗涉及阴囊处复杂性肛瘘的临床疗效。方法:将80例涉及阴囊处复杂性肛瘘患者随机分为2组各40例,治疗组行改良的肛瘘切除术,术后阴囊处创面缝合配合中医定向透药治疗,对照组行传统肛瘘切除术,术后阴囊处创面开放及常规治疗。比较2组疗效及术后平均创面愈合时间、疼痛情况、术后并发症发生率。结果:2组总有效率均为100%,差异无统计学意义(P>0.05);治疗组平均创面愈合时间、疼痛评分及创面渗出量、渗血量评分均优于对照组,术后肛缘水肿、肛门畸形、肛周湿疹等并发症发生率低于对照组,差异均有统计学意义(P<0.05)。结论:改良手术方式加中医定向透药疗法治疗涉及阴囊处复杂性肛瘘具有较好疗效。Objective：To investigate the clinical effect of modified surgical procedure combined with traditional Chinese medicine（TCM） targeted transdermal drug delivery in the treatment ofcomplex anal fistula involving the scrotum.Methods：A total of 80 patients with complex anal fistula involving the scrotum were randomly divided into treatment group and control group,with 40 patients in each group.The patients in the treatment group were given modified resection of anal fistula,followed by suture of the wound in the scrotum combined with TCM targeted transdermal drug delivery,and those in the control group were given conventional resection of anal fistula,followed by conventional treatment with the wound in the scrotum kept open.The two groups were compared in terms of meanwound healing time after surgery,pain score,and the incidence rate of postoperative complications.Results：Both groups had an overall response rate of 100% and there was no significant difference between the two groups（P0.05）.Compared with the control group,the treatment group had significantly better meanwound healing time,pain score,and score of wound exudation and significantly lower incidence rates of postoperative complications including anal verge edema,anal deformity,and perianal eczema（P0.05）.Conclusion：Modified surgical procedure combined with TCM targeted transdermal drug delivery has a good clinical effect in the treatment of complex anal fistula involving the scrotum.
