腰骶丛MRI形态学成像在诊断CIDP患者中的价值

摘　　要：目的:探索CIDP患者MRI的腰骶丛神经形态学变化在临床诊断中的价值。方法:使用3.0T MR对16名CIDP患者及25名健康志愿者进行腰骶椎常规MRI扫描,以及FS 3D MERGE、STIR序列扫描。通过腰骶丛神经冠状位MIP图像,分别测量腰4~骶2双侧神经的直径。健康对照组及CIDP组所测神经根粗细值进行两独立样本t检验及ROC曲线分析。结果:健康对照组L4神经根直径4.07±0.70mm,L5神经根直径4.6±0.88mm,S1神经根直径4.35±0.50mm。CIDP组L4神经根直径5.06±1.38mm,L5神经根直径6.1±1.02mm,S1神经根直径5.58±1.40mm。两组L4、L5及S1神经根直径比较具有统计学意义(P<0.001)。ROC曲线分析结果:曲线下面积(AUC)为0.741,临界值为4.47mm,敏感度及特异度分别为68.3%和81.0%。结论:MRI腰骶丛神经根成像对CIDP患者神经根形态学改变具有诊断意义,4.47mm可以作为诊断CIDP患者腰骶丛神经增粗的临界值。Purpose： To explore the value of lumbosacral plexus MRI in clinical diagnosis of CIDE Methods： A total of 16 CIDP patients and 25 healthy volunteers underwent lumbosacral vertebrae MR scanning by 3.0T MR scanner including FS 3D MERGE and STIR sequences. Through the lumbosacral plexus coronal MIP images, the diameters of bilateral lumbar 4-scaral 2 nerve were measured. Healthy control group and CIDP group were compared by two independent t-test and ROC curve analysis was performed. Results： In the healthy control group, the diameter of 4th the lumbar nerve, 5th lumbar nerve and I st sacral nerve was 4.07~0.70mm, 4.6＋0.88mm and 4.35＋0.50mm. The diameter of the 4th lumbar nerve, 5th lumbar nerve and 1st sacral nerve in the CIDP group was 5.06＋1.38ram, 6.1~1.02 mm and 5.58 ^-l.40mm. The difference between the two groups was with statistical significant （P〈0.001）. The area under the ROC curve （AUC） was 0.741. The cut off value was 4.47mm. The sensitivity and specificity were 68.3% and 81% respectively.Conclusion： MRI lumbosacral plexus root imaging has a diagnostic significance for nerve root lesions in patients with CIDE 4.47rnm can be used as a diagnostic threshold for the diagnosis of lumbosacral plexus hypertrophy in patients with CIDE
