清胆和胃颗粒治疗胆汁反流性胃炎47例临床观察

摘　　要：目的观察清胆和胃颗粒治疗胆汁反流性胃炎的临床疗效。方法将93例胆汁反流性胃炎患者随机分为治疗组47例,对照组46例。治疗组给予清胆和胃颗粒,每次10 g,每日2次,餐后2 h口服;对照组给予复方铝酸铋颗粒,每次1.3 g,每日3次,餐前30 min口服。两组疗程均为4周。治疗前后进行中医症状评分、胃黏膜病理组织检测,治疗后评价中医症状疗效。结果治疗后治疗组患者胃胀、胃痛、反酸、烧心、纳呆、口苦、口臭、心烦易怒、尿短黄、便秘等各项中医症状评分均较治疗前明显降低,且胃胀、纳呆、口苦、口臭、心烦易怒症状评分明显低于对照组(P<0.05)。治疗组中医症状疗效总有效率为87.2%,明显高于对照组的80.4%(P<0.05)。两组复查胃镜患者(治疗组32例、对照组29例)治疗后固有层水肿、血管扩张、炎症积分较治疗前均有下降,且治疗组较对照组降低明显(P<0.05)。结论清胆和胃颗粒治疗胆汁反流性胃炎不仅能明显改善患者中医症状,同时能减轻局部炎症反应。Objective To observe the clinical effect of Qingdan Hewei Granules（ 清胆和胃颗粒） on bile reflux gastritis. Methods Randomly divided 93 patients with bile reflux gastritis into treatment group with 47 cases,and control group with 46 cases. The treatment group was given Qingdan Hewei Granules orally,10 g each time,twice a day,2 h after meals; the control group was given Compound Bismuth Aluminate Granules orally,1. 3 g each time,3 times a day,30 min before meals. Both groups were treated for 4 weeks. Before and after treatment,graded the Chinese medicine symptoms,detected pathological tissue of gastric mucosa,and evaluated the curative effect of Chinese medicine symptoms after treatment. Results After treatment,the Chinese medicine symptom scores including bloating,stomach pain,acid regurgitation,heartburn,anorexia,bitter taste,ozostomia,vexation and irritability,short yellow urine,and constipation of the treatment group patients were significantly lower than those before treatment,and symptom scores of bloating,anorexia,bitter taste,ozostomia,vexation and irritability were significantly lower than the control group（ P 0. 05）. The total effective rate of curative effect on Chinese medicine symptoms in the treatment group was 87. 2%,which was significantly higher than that of the control group of 80. 4%（ P 0. 05）.The level of lamina propria,vasodilatation and inflammation of patients reexamining gastroscopy in both groups（ 32 cases in the treatment group,29 cases in the control group） all decreased after treatment compared with those before treatment,and the treatment group decreased more significantly than the control group（ P 0. 05）. Conclusion Qingdan Hewei Granules in the treatment of bile reflux gastritis could not only improve symptoms,but also reduce local inflammatory response.
