摘 要:目的:研究麻疹病毒感染患者的肝功能损伤程度与外周血淋巴细胞亚群表达率的相关性及其防治对策。方法:选取2015年5月—2017年5月间收治住院的麻疹患者肝功能异常者380例临床资料,将其分为小儿组110例和成人组270例;另选取2013年5月—2017年5月间收治的住院麻疹患者420例临床资料,将其分为肝功能正常组209例和肝功能异常组211例;再选取同期医院体检中心接受体检的健康成人为正常对照组200例;采用全自动生化分析仪检测各组麻疹病毒感染患者的肝功能各指标;另采用流式细胞仪测定麻疹病毒感染患者血清中淋巴细胞亚群(CD4+、CD8+、CD4+/CD8+、CD19+(B)、CD16+/56(NK)水平值,分析各组麻疹病毒感染患者的肝功能损伤程度与外周血淋巴细胞亚群表达率的相关性。结果:麻疹病毒感染患者中成人组ALT升高的幅度高于少儿组(P<0.01)。成人组麻疹患者中肝功能正常组、肝功能异常组的总T淋巴细胞、CD4+、CD4+/CD8+测得值均低于对照组(P<0.05);肝功能异常组患者CD8+、CD4+/CD8+测得值低于肝功能正常组(P<0.05),而B细胞和NK细胞检测值3组间经比较其差异无统计学意义((P<0.05)。结论:麻疹病毒感染人群中成人出现肝功能损害的发生率高于小儿,成年人中麻疹患者肝脏损伤程度与外周血淋巴细胞分化抗原中T细胞亚群表达率具有相关性,因此调节细胞免疫对麻疹患者肝损伤的治疗具有一定临床意义。Objective: To study the correlation between the injury degrees of liver and the expression rates of lym- phocyte subsets among measles virus infected population and its control strategies. Methods: The clinical data of 380 cases of abnormal liver function of measles patients admitted to hospital from May 2015 to May 2017 were selected. They were divided into pediatric group (110 cases) and adult group (270 cases). Another 420 cases of adult measles pa- tients admitted to hospital between May 2013 and May 2017 were divided into normal liver function group (209 cases) and abnormal liver function group (211 cases). And then another 200 cases who received physical examination in the hospital center were selected as the normal control group. Liver timction indicators were detected by using the automat- ic biochemical analyzer. The levels of lymphocyte subsets (CD4+, CD8+, CD4+/CD8+, CD19+ (B) and CD16+/56+ (NK) in the serum of patients with measles virus infection were determined by flow cytolnetry. Then was analyzed the correlation between the degree of liver injury and the expression rates of peripheral blood lymphocyte subsets in pa- tients with measles virus infection of each group. Results: According to the data, the increased rate of ALl" (Alanine transaminase) in the group of adults was significantly higher than that of the group of pediatric (P〈O.OI) among mea- sles virus infected participants. In addition, the measured value of total T-cells, CD4+ cells and CD4+/CD8+ amongmeasles virus infected adult patients (both with and without normal liver function) were lower than that ot the control adult group (P〈 0.05). Compared with the measles virus infected patients with normal liver function, the measured val- ue of CDS+, CD4+/CD8+ of the patients with abnormal liver function was significantly decreased (P〈 0.05). But there was no obvious difference among three groups in the measured value of B cells and NK cells. Conclusion:
