摘　　要：目的:评价鸦胆子油乳联合放疗治疗脑转移瘤的疗效和安全性。方法:计算机检索中国知网CNKI、万方数据库、中国生物医学文献数据、Pubmed、EMBASE、Cochran Library、SCI,检索时间从建库截止至2015年11月,并补充使用Google Scholar等搜索引擎在互联网上查找相关文献,纳入有关鸦胆子油乳联合放疗治疗脑转移瘤的随机对照试验,进行文献质量评价,主要结局指标包括疾病缓解率、中位生存时间、生活质量、免疫功能及治疗相关不良反应,采用相对危险度(relative risk,RR)为效应量,各效应量以95%置信区间(95%CI)表示,用Rev Man5.3进行Meta分析。结果:共纳入7项合格研究,共486例病例。Meta分析提示:在脑转移瘤的治疗上,单纯放疗与鸦胆子油乳联合放疗相比,后者可有效提高疾病缓解率;能更好地改善患者的生活质量;能保护患者的免疫功能;治疗组骨髓抑制和胃肠道反应的发生率明显低于对照组。结论:鸦胆子油乳能提高脑转移瘤患者的放疗效果,改善生活质量,保护免疫功能,减轻放疗产生的不良反应。Objective： To evaluate the clinical efficacy and safety of Brucea javanica oil emulsion combined with radiotherapy for intracranial metastatic tumor. Methods： The CNKI, Wanfang, CBM, Pubmed, EMBASE, Cochran Library, SCI were retrieved via computer. The retrieval time was from when the databases were built to November, 2015. Search engines on the Internet such as Google Scholar were used as well. All randomized controlled clinical studies on Brucea javanica oil emulsion combined with radiotherapy for intracranial metastatic tumor were included. Two researchers extracted data and evaluated the quality of the studies respectively. The outcomes include disease response rate, median survival time, life quality, immune function and treatment for the related negative reaction. The data was analyzed by RevMan 5.3. Results： Seven trials and 486 intracranial metastatic tumor patients were included. Meta-analysis indicates that compare with radiotherapy, Brucea javaniea oil emulsion combined with radiotherapy could increase patients＂ disease response rate, improve quality of life, protect immune function, as well as significantly lower the incidence of rest of bone marrow and gastroenteric reaction. Conclusion： Brucea javanica oil emulsion could increase the effect of radiotherapy, improve the quality of patients life, protect immune function and lighten the radiotherapy related negative reactions.
