痛经贴合血府逐瘀颗粒对原发性痛经寒凝血瘀证患者血清PGE_2、PGF_(2α)的影响

摘　　要：目的:探讨血府逐瘀颗粒联合痛经贴治疗原发性痛经寒凝血瘀证的临床疗效及机制。方法:选取原发性痛经寒凝血瘀证患者222例,随机分为联合治疗组58例,痛经贴组56例,血府逐瘀组55例,西药对照组53例。痛经贴组在关元穴贴痛经贴,血府逐瘀组内服血府逐瘀颗粒,联合治疗组为痛经贴组与血府逐瘀组治疗叠加,西药对照组内服布洛芬缓释胶囊,各组均连续治疗3个月经周期。观察各组疗效、痛经症状评分、外周血血清PGE_2、PGF_(2α)水平。结果:联合治疗组疗效优于其他治疗组(P<0.01),联合治疗组痛经症状评分较其他治疗组显著下降(P<0.01),联合治疗组对外周血PGE_2、PGF_(2α)水平的影响优于其他治疗组(P<0.01)。结论:痛经贴联合血府逐瘀颗粒治疗原发性痛经寒凝血瘀证疗效确切,优于单用痛经贴或血府逐瘀颗粒,其机制可能与调节外周血PGE2、PGF2α水平有关。Objective： To study the clinical effect and mechanism of Tongjingtie combined with Xuefu Zhuyu Granule in treatment of primary dysmenorrhea of cold coagulation and blood stasis. Method： A total of 222 patients with primary dysmenorrheal of cold coagulation and blood stasis were randomly divided into combined treatment group （58 cases）, Tongjingtie group （56 cases）, Xuefu Zhuyu Granule group （55 cases） and control group （53 cases）. Patients treated by Tongjingtie on ＂Guanyuan （CV4）＂ in Tongjingtie group, oral administration of Xuefu Zhuyu Granule IN Xuefu Zhuyu Granule group, Tongjingtie as well as Xuefu Zhuyu Granule in combined treatment group, and oral administration of ibuprofen sustained-release capsules in the control group. All the patients were treated for two menstrual cycle in the three groups. The efficacy, dysmenorrhea symptoms score, the serum level of PGE2 and PGF2α were observed in the three groups. Results： The combined treatment group showed better efficacy tan the other groups （P〈0.01）, as well as lower dysmenorrhea symptom scores than the the other groups （P〈0.01）. The combined treatment group showed better effect than the other groups on the serum level of PGE2 and PGF2α （P〈0.01）. Conclusion： Tongjingtie combined with Xuefu Zhuyu Granule showed exact effect on primary dysmenorrhea of cold coagulation and blood stasis. The mechanism may be related to the effect of adjusting the serum level of PGE2 and PGF2α.
