摘  要:目的制订患有基础疾病儿童的预防接种建议,并评估预防接种安全性。方法组织临床儿科、流行病学、疫苗学等相关领域专家论证,形成基础疾病儿童的预防接种共识建议,观察符合接种建议的患儿接种疫苗后30天内的不良反应。结果制订了14种疾病的预防接种专家共识建议,用于北京市顺义区26家常规接种门诊。对患免疫性血小板减少性紫癜、惊厥、先天性甲状腺功能减退或亢进、先天性胆道闭锁、川崎病的25名儿童进行了147剂次的疫苗接种,均未发生不良反应。结论采用专家论证形成预防接种共识建议的方法可指导接种医生对患病儿童接种疫苗,提高接种率,确保接种安全。Objective To develop immunization recommendations for children with underlying medical conditions and assess the safety of the recommendations. Methods We organized a group of experts,including pediatricians,epidemiologists,and vaccine experts,to develop recommendations for immunizing children with underlying medical conditions. We monitored adverse reactions for 30 days after children with underlying medical conditions were vaccinated. Results Fourteen immunization recommendations were developed and were used in 26 immunization clinics of Shunyi district in Beijing. Using these recommendations,147 doses of various vaccines were administered to 25 children with either thrombocytopenic purpura,a convulsive disorder,congenital hypothyroidism,congenital hyperthyroidism,congenital biliary atresia,or Kawasaki disease. No adverse reactions were reported. Conclusions Committees of experts can develop immunization recommendations and can provide vaccinators with appropriate guidelines to safely immunize children with underlying medical conditions. The recommendations can help to ensure high vaccine coverage while providing individual protection of vulnerable children from vaccine preventable diseases.
