补阳还五汤加味辨证内服联合复方黄柏液外洗治疗糖尿病足溃疡56例临床疗效观察

摘　　要：目的:观察补阳还五汤加味辨证内服联合复方黄柏液外洗治疗糖尿病足溃疡56例临床疗效。方法:选取我院在2014年1月~2016年7月期间收治的112例糖尿病足溃疡患者,按照不同治疗方式将患者分成两组(每组56例),对照组采用复方黄柏液外洗治疗,观察组患者采用补阳还五汤加味辨证内服联合复方黄柏液外洗治疗。比较两组患者临床治疗疗效以及两种治疗方式对患者血管内皮功能的影响。结果:治疗后观察组患者总有效率明显高于对照组[(91.07%)vs(71.43%)](P<0.05);治疗前两组患者中医证候、创面面积、疼痛程度评分结果比较无差异(P>0.05),治疗后观察组患者中医证候、创面面积、疼痛程度评分结果明显低于对照组(P<0.05);治疗前两组患者ET、TXB2与NO表达水平比较无差异(P>0.05),治疗后ET、TXB2均有下降,但观察组患者下降程度明显高于对照组(P<0.05),而NO表达水平明显升高,并且观察组患者升高程度明显高于对照组(P<0.05);两组患者未出现严重的不良反应情况。结论:补阳还五汤加味辨证内服与复方黄柏液外洗联合治疗糖尿病足部溃疡患者临床疗效显著,能促进血管内皮功能的恢复及缓解患者疼痛,并且无不良反应情况,值得在临床上推广使用。Objective： To study the clinical effect of using Modified Buyang Huanwu Decoction syndrome differentiation com- bined with Compound Phellodendron Liquid external washing in the treatment of 56cases of diabetic foot ulcer. Methods： 112patients with diabetic foot ulcer were enrolled in our hospital from January 2014to July 2016. Patients were divided into two groups （56cases in each group） according to different＇treatment methods. The controlled group was treated with Compound Phel- lodendron Liquid. The observation group was treated with Modified Buyang Huanwu Decoction syndrome differentiation combined with Compound Phellodendron Liquid external washing. To compare the clinical efficacy of the two groups of patients and the effect of the two treatments on the vascular endothelial function of the patients. Results ： After treatment, the total effective rate of the observation group was significantly higher than that of the controlled group [ （91.07%） vs （71.43%） ] （P〈0. 05） ; the two groups of patients before treatment, TCM syndrome, wound area; （P〈0. 05）. There was no significant difference in ET, TXB2and NO expression between the two groups before and after treatment （ P〈0. 05 ）. There was no significant difference be- tween the two groups （P〉0. 05）. There was no significant difference between the two groups （P〉0. 05）, while the level of NO expression was significantly higher than that of the controlled group （P〈0. 05 ）, and the level of NO was significantly higher in the observation group than in the controlled group （P〈0. 05 ）. The two groups of patients did not appear serious adverse reactions. Conclusion ： Modified Buyang Huanwu Decoction syndrome differentiation combined with Compound Phellodendron Liquid external washing is effective in the treatment of diabetic foot ulcer. It can promote the recovery of vascular endothelial function and relieve pain and no adverse reaction. It is worthy of clinical promotion.
