村医培训在提高少数民族地区农村老年高血压治疗依从性中的效果研究

摘　　要：目的分析村医培训在提高少数民族地区农村老年高血压治疗依从性中的效果。方法采用整群随机抽样方法,抽取宁夏某县原发性高血压患者作为研究对象,分别于2015年8月和2016年8月进行基线调查和干预后调查,基线调查后进行村医高血压知识专项培训,督促村医利用一年4次的高血压随访机会,对患者进行面对面高血压健康教育。分析村医专项培训对提高高血压患者治疗依从性的效果。结果 429名患者进入基线调查,干预一年后,随访到398名,失访率为7.22%。村医培训干预后,患者遵医行为、不良服药行为、日常生活管理行为和总依从性得分均有所提高(P<0.05)。而烟酒嗜好管理行为和高血压知识水平与干预前相比较,差异无统计学意义(P>0.05)。结论对少数民族地区村医的专项培训能明显提高老年高血压患者的治疗依从性。Objective To study the effect on improving treatment compliance of elderly patients with hypertension in minority rural areas through village doctors＇training. Methods Elderly patients with primary hypertension in Ningxia were selected as the research objects using cluster random sampling method. Baseline surveys and after intervening surveys were conducted in August 2015 and August 2016, respectively. After baseline surveys, a special training for village doctors was performed. The village doctors were urged to operate hypertension knowledge propaganda and face to face health education using 4 times follow - up opportunities a year. The effect of special training for village doctors on treatment compliance of patients with hypertension was analyzed. Results The effective data of 429 cases of elderly patients with hypertension were collected in the baseline survey. After intervening for one year,398 patients were followed up, and the rate of loss to follow - up was 7.22%. After training, the scores of patients＇ behaviour of medication adherence, the behavior of medication non - adherence, the behavior of daily life management and total therapeutic adherence were raised compared with before convening （ P 〈 0. 05 ）. Scores of the behaviour of smoking and drinking hobby management and hypertension knowledge level had no statistical significance（ P 〉 0. 05 ）. Conclusion Special training for village doctors can significantly improve the treatment compliance of elderly patients with hypertension.
