乳管镜联合超声检查诊断乳管内病变的价值研究

摘　　要：目的探讨乳管镜联合超声检查诊断乳管内病变的价值,为临床早期诊断、早期治疗提供借鉴。方法选取哈励逊国际和平医院2013年3月—2015年9月收治的119例乳头溢液患者,分别行乳腺超声、乳管镜及乳管镜联合超声检查,并将检查结果与乳管内病变组织病理检查结果进行比较分析。结果乳管镜联合超声检查诊断乳管内乳头状癌、乳头状瘤、乳腺导管癌、乳腺炎的符合率均高于乳管镜、超声检查(P<0.05)。乳管镜联合超声检查诊断乳管内病变良恶性的正确率高于乳管镜、乳腺超声(P<0.05)。3种乳头溢液中乳头状癌、乳头状瘤、乳腺导管癌、乳管扩张症和乳腺炎分布比较,差异无统计学意义(P>0.05)。结论乳管镜联合超声检查诊断乳管内病变优于单独应用乳管镜或超声检查,对乳管内病变组织良恶性有较高的鉴别诊断价值。Objective To explore the value of breast duct endoscopy combined with breast ultrasonography in the diagnosis of intraductal lesions,in order to provide a reference for early diagnosis and treatment of this kind of disease. Methods We enrolled 119 cases of nipple discharge receiving treatment in Harrison International Peace Hospital from March 2013 to September 2015. Breast ultrasonography,breast duct endoscopy,and breast duct endoscopy combined with breast ultrasonography were preformed in them respectively,and the results were compared with the pathological results of intraductal lesions. Results Breast duct endoscopy combined with breast ultrasonography had higher accuracy than breast duct endoscopy, breast ultrasonography,alone in the diagnosis of intraductal papillary carcinoma,papilloma,breast ductal carcinoma and mastitis（ P 0. 05）. Breast duct endoscopy combined with breast ultrasonography had better performance in differentiating benign intraductal lesions from malignant ones compared with the other two examinations（ P 〈 0. 05）. However, these three examinations had no significant differences in identifying papillary carcinoma,papilloma,breast ductal carcinoma among patients with yellow nipple discharge or those with bloody nipple discharge（ P 〈 0. 05）. Conclusion Breast duct endoscopy combined with breast ultrasonography is superior to other two examinations in both identifying breast intraductal lesions and differentiating benign intraductal lesions from malignant ones.
