摘  要:目的:探讨以基底动脉为监测层面行脑动脉CT血管成像(HCTA)扫描技术的可行性。方法:对临床申请HCTA扫描的138例检查者,随机分为两组各69例,用Time Bolus法获得时间-密度曲线,分别测量大脑中动脉和基底动脉达峰时间,并分别以达峰时间计算HCTA实际扫描延时时间;对所得HCTA影像进行评价,分析大脑中动脉和基底动脉所测达峰时间是否都能用于HCTA扫描技术。结果:对所有138例检查者均测量大脑中动脉和基底动脉时间-密度曲线,大脑中动脉达峰点均值为6.427 5,实际延时时间17.82 s;基底动脉达峰点均值为6.630 4,实际延时时间18.21 s,两种测量方法皮尔逊相关系数为0.92,表明相关性较高。线性回归分析的回归方程为:大脑中A测量点数=1.035×基底A测量点数-0.424 4,R方为0.841 8,回归方程的方法分析F值为723.557,P值为2.64×10^(-56),回归方程有统计学意义;大脑中A实际延时时间=1.040×基底A实际延时时间-1.112 3,R方为0.839 9,回归方程的方法分析F值为713.652,P值为5.81×10^(-56),回归方程有统计学意义。两组图像质量的主观评分结果采用卡方检验,卡方值=0.163,P=0.687,认为两组评分基本一致。结论:在检查者体位不适扫描状态及大脑中动脉显示不佳的情况下,以基底动脉为监测兴趣区可以达到常规HCTA以大脑中动脉为监测兴趣区相同的效果,且两种测量方法相关性较高,图像质量符合诊断要求。可以为脑血管疾病患者得到及时、成功地完成HCTA扫描,提供可靠的技术保证。Objective: To discuss the feasibility of HCTA scanning with basilar artery as the monitoring level.Methods:One hundred and thirty-eight cases of clinical application of HCTA scanning were randomly divided into two groups,69 cases in each group.The time density curve was obtained by Time Bolus method.The peak time of middle cerebral artery and basilar artery was measured respectively,and the actual delay time of HCTA scanning was calculated based on the peak time The HCTA images results have been evaluated to analyze whether there are difference for the peak time between middle cerebral artery and basilar artery,and whether they can be used to HCTA scanning technology.Results: The timedensity curve of the middle cerebral artery and basilar artery of 138 patients was measured.The average tmax of middle cerebral artery was 6.427 5,and the actual delay time was 17.82 s; The average tmax of basal artery was 6.630 4,and the actual delay time was 18.21 s.The two methods of measurement are highly correlated with Pearson correlation coefficient of 0.92.The regression equation of linear regression analysis was as follows: the number of measuring points of Middle cerebral artery = 1.035 × the number of measuring points of basilar artery-0.424 4; R was 0.8418.F was 723.557.P was 2.64 × 10-56.The regression equation showed statistical significance.The actual delay time of middle cerebral artery= 1.040 × the actual delay time of basal artery-1.112 3.R was 0.839 9.F was 713.652.P was 5.81 × 10-56.The regression equation showed statistical significance.Chi-squared test was used to determine the subjective score of image quality of the two groups.The chi-square value was 0.163.P was 0.687.The results showed that the scores of the two groups were basically identical.Conclusions: When the examinee is in a state of discomfort scanning and the middle cerebral artery is poorly displayed,basal artery monitoring interest area can achieve the same effect as conventional HCTA middle cerebral artery monitoring interest area d
