尿道海绵体成形对尿道下裂再造尿道功能影响的实验研究

摘　　要：目的:探究尿道海绵体成形对尿道下裂修复术再造尿道功能的影响。方法:14只先天性尿道下裂兔随机分成两组,实验组7只采用Duplay+尿道海绵体成形术进行尿道下裂修复;对照组7只仅用Duplay术式修复。术后4周时进行在体再造尿道顺应性检测(测量一段再造尿道内充注空气膨胀时的压力)及离体再造尿道标本尿流率检测。比较两组术后再造尿道的功能。结果:全部动物均按计划实施手术及检测操作。实验组压力平均测值略高于对照组,但两组间比较差异无统计学意义。两组间尿流率比较差异无统计学意义。结论:在尿道下裂修复术中实施或未实施尿道海绵体成形,术后再造尿道的功能无明显差异。Objective： To investigate the effects of spongioplasty on neourethral function after hypospadias repair.Methods： A total of 14 congenital hypospadiac New Zealand male rabbits were randomly allocated into 2 groups.Seven animals of experimental group underwent Duplay hypospadias repair and spongioplasty; another 7 for control group underwent Duplay surgery only.Four weeks after surgery,compliance test of the reconstructed urethra was performed in vivo（by measuring intraluminal pressure of an isolated segment of reconstructed urethra after progressively distended with air）,and uroflormetry test in vitro,so as to implement functional comparison of the two groups.Results： All animals received planned procedures.Mean pressures tended to be higher in the experimental groups than in control group without reaching the statistical significance.Uroflowmetry test of the two groups also showed no significant difference.Conclusion： There is no obvious functional difference between reconstructed urethrae after hypospadias repair with and without spongioplasty.
