摘 要:目的观察妊娠期糖尿病(GDM)孕妇肘静脉血清、新生儿脐静脉血清中晚期糖基化终末产物(AGE)及AGE受体(RAGE)水平的变化,探讨其二者与GDM发病、新生儿早期神经系统发育的关系。方法选取GDM孕妇80例(GDM组),按孕周匹配同期住院分娩糖耐量正常孕妇80例为对照组。采用ELISA法分别检测两组母体肘静脉及新生儿脐静脉血清中AGE、RAGE水平;出生72 h内,对新生儿行新生儿神经行为测定(NBNA)评分;并对上述指标之间进行相关分析。结果 GDM组母体肘静脉血清及新生儿脐静脉血清中AGE、RAGE水平高于对照组(P均<0.05)。GDM组新生儿主动肌张力、原始反射及NBNA总分高于对照组(P均<0.05)。GDM组母体肘静脉血清中AGE、RAGE水平分别与新生儿脐静脉血清中AGE、RAGE水平呈正相关性(r分别为0.725、0.473,P均<0.05)。GDM组母体肘静脉血清中AGE、RAGE水平与NBNA评分呈负相关性(r分别为-0.415、-0.320,P均<0.05)。GDM组新生儿脐静脉血清中AGE、RAGE水平与NBNA评分呈负相关性(r分别为-0.294、-0.280,P均<0.05)。结论 GDM母体肘静脉血清及新生儿脐静脉血清中AGE、RAGE水平升高,其与GDM发生及新生儿早期神经系统缺陷均有关。Objective To investigate the changes in levels of advanced glycation end products( AGE) and its receptor( RAGE) in the venous and umbilical vein serum of pregnant women with gestational diabetes mellitus( GDM),and its relationships with the onset of GDM and the development of early neonatal nervous system. Methods Eighty cases of GDM pregnant women( group GDM) were selected,and 80 cases of pregnant women with normal glucose tolerance in the same period of hospital delivery and the same gestational age were selected as the control group. The levels of AGE and RAGE in the maternal and neonatal umbilical vein serum were detected by ELISA in two groups. The neonatal behavioral neurological assessment( NBNA) scores were measured at 72 h after neonatal birth. The correlation between these indexes was analyzed. Results The levels of AGE and RAGE in the maternal serum and neonatal umbilical vein serum of the GDM group were higher than those in the control group( all P 〈0. 05). The neonatal active muscle tension,primitive reflex,and NBNA total score were higher in the GDM group than in the control group( all P 〈0. 05). The levels of AGE and RAGE in the maternal serum of the GDM group were positively correlated with AGE and RAGE levels in the umbilical vein serum of neonates( r = 0. 725,0. 473; both P〈 0. 05). The levels of AGE and RAGE in the maternal serum of the GDM group were negatively correlated with NBNA score( r =-0. 415,-0. 320; both P〈 0. 05). The levels of AGE and RAGE in the neonatal umbilical vein serum of the GDM group were negatively correlated with NBNA score( r =-0. 294,-0. 280; both P 〈0. 05). Conclusion The levels of AGE and RAGE in the GDM maternal vein serum and neonatal umbilical vein serum increase,which are related to the occurrence of GDM and the early neurological deficits of newborns.
