三级甲等医院急诊分诊护士预检分诊决策能力现状及相关因素研究

摘　　要：目的调查北京市急诊分诊护士预检分诊决策能力,探索其相关因素,对构建急诊分诊护士岗位培训体系提供指导意见。方法抽取北京市15所三级甲等医院206名急诊分诊护士采用一般资料调查表和预检分诊决策量表进行问卷调查。结果护士预检分诊决策能力总分(149.53±30.77)分,各维度得分由高到低依次为认知行为、经验及技术信心、直觉、评判性思维。职称、进修级别、急诊相关证书、参加病例讨论是护士预检分诊决策能力的影响因素(P<0.01或P<0.05)。结论北京市15所三级甲等综合医院急诊分诊护士预检分诊决策能力整体水平低,预检分诊考核系统不完善。提示护理管理者可通过进修、组织病例讨论等方法提高护士评判性思维能力,进而提高护士的预检分诊决策能力。Objective To investigate the current situation and related factors of pre-examination and triage decision-making ability of emergency nurses in Beijing,in order to provide guidance for the construction of emergency triage nurses training system. Methods Totally,206 emergency nurses in 15 3 A-level general hospitals in Beijing were surveyed by general data questionnaire and pre-examination decision scale. Results The total score of the pre-examination and triage decision-making ability was（ 149. 53 ± 30. 77）. The scores from high to low were cognitive behavior,experience technical confidence,intuition and critical thinking. Professional title,further education level,emergency-related certificate,participation in case discussion were the factors influencing nurses＇ triage decision-making ability（ P〈0. 01 or P〈0. 05）. Conclusion The overall level of pre-examination and triage decision-making ability in 15 3 A-level general hospitals in Beijing is low,and the pre-examination assessment system is imperfect. It is suggested that nurses can improve the ability of nurses＇ critical thinking through training,and organization of case discussion,to improve nurses＇ ability of pre-screening decision-making.
