经阴道彩色超声在宫外孕保守治疗中应用的评价

摘　　要：目的研究经阴道彩色超声在宫外孕保守治疗中的应用。方法选择2015年12月-2016年12月医院收治的宫外孕患者58例,均采用保守治疗的方法,同时利用经阴道彩色超声检查的方法,动态监测患者保守治疗过程。对所有患者临床资料进行回顾性分析,统计保守治疗结果,分析盆腔积液、血流信号、包块类型、包块大小、血人绒毛膜促性腺激素(β-HCG)与治疗效果之间的关系,并总结经阴道彩色超声检查在宫外孕保守治疗中的应用价值。结果 58例患者保守治疗成功40例,成功率为68.97%,其余18例保守治疗失败后转为手术治疗。盆腔积液、血流信号、包块类型、包块大小、血β-HCG等指标,在保守治疗成功组与失败组间差异均具有统计学意义(P<0.05)。结论在宫外孕患者保守治疗中,采用经阴道彩色超声检查的方法,能够提高患者疗效评价结果,同时帮助确定治疗方案,提升治疗效果,具有重要的临床应用价值。Objective To explore the application of transvaginal color doppler ultrasound in conservative treatment of ectopic pregnancy. Methods From December 2015 to December 2016, a total of 58 ectopic pregnancy patients were recruited from Hunan provincial maternal and children＇ s health care hospital. All of these patients were treated by conservative approach. At the same time, the process of the conservative treatment of the patients was dynamically observed by using the transvaginal color doppler ultrasound. All of the clinical data of the patients were analyzed retrospectively. The contents of the statistical analysis included the results of conservative treatment, the relationship between the pelvic cavity accumulates liquid, blood flow signal, types and size of masses, blood human chorionic gonadotropin （HCG） and effect of therapy. Moreover, the application value of transvaginal color doppler ultrasound in conservative treatment of ectopic pregnancy was summarized. Results Of 58 patients treated by conservative approach, 40 cases was treated successfully. The treatment success rate was 68.97%. The other 18 cases were treated by surgery after failure of conservative treatment. There was statistical difference in pelvic cavity accumulates liquid, blood flow signal, types and size of masses, blood HCG between the success group and the failure group （P 〈 0.05）. Conclusions The application of transvaginal color doppler ultrasound in conservative treatment of ectopic pregnancy can improve the evaluation results of patients curative effect. It has important clinical value in both helping to decide the treatment protocol and improving the effect of therapy.
