闭合复位克氏针内固定结合双柏散外敷治疗小儿移位性肱骨髁上骨折

摘　　要：目的观察闭合复位克氏针内固定结合双柏散外敷治疗小儿移位性肱骨髁上骨折的临床治疗疗效。方法按照随机数字分配原则选用在本院进行治疗移位性肱骨髁上骨折的80例患儿作为研究对象进行回顾性分析,并将所有患儿进行分组治疗,即观察组与对照组,每组患儿各40例。给予观察组患儿的治疗方法为使用闭合复位克氏针内固定结合双柏散外敷治疗进行医治,而对照组则是单独使用切开复位内固定进行治疗。观察比较2组患儿肘内翻畸形、Volkmann挛缩的发生率以及关节功能的优良率。结果治疗结束后,观察组的优良率为97.50%,对照组的优良率为80.00%,相比对照组,观察组的优良率有显著提升,差异具有统计学意义(P<0.05);而在治疗过程中,观察组的肘内翻畸形、Volkmann挛缩的发生率为5.00%,对照组的肘内翻畸形、Volkmann挛缩的发生率为25.00%,相比对照组,观察组的肘内翻畸形、Volkmann挛缩的发生率比较低,差异具有统计学意义(P<0.05)。结论闭合复位克氏针内固定结合双柏散外敷治疗小儿移位性肱骨髁上骨折具有极高的临床应用价值,其临床效果不仅显著提高,并且患儿的并发症发生率也比较低,患儿的临床症状得到有效的改善,此法值得在小儿移位性肱骨髁上骨折的临床治疗中推广使用。Objective To observe the clinical effect of displaced humeral supracondylar fractures in children treated with closed reduction and Kirschner wire internal fixation combined with external application of Shuangbai powder.Methods According to the random number distribution principle for treatment,80 cases with displaced humeral supracondylar fractures were selected as the research objects.All the children were divided into group treatment,that is,the observation group and the control group,40 cases in each group.The observation group was given closed reduction and Kirschner wire internal fixation combined with external application of Shuangbai powder.The control group was given closed reduction and Kirschner wire internal fixation.The incidence of Volkmann contracture and the good rate of joint function were observed and compared between the 2 groups.Results The excellent rate of the observation group was 97.50%,the excellent rate of control group was 80%,comparing with the control group,the excellent rate of the observation group significantly improved（P0.05）.In the course of treatment,cubitus varus Volkmann contracture incidence rate of the observation group was 5%,cubitus varus Volkmann contracture incidence rate of the control group was 25%,comparing with the control group,Volkmann elbow varus contracture incidence of the observation group was relatively low,and the difference was statistically significant（ P〈0.05）.Conclusion Closed reduction and Kirschner wire internal fixation combined with external application of Shuangbai powder in the treatment of displaced humeral supracondylar fractures in children has high clinical application value,its clinical effect is not only significantly high,and children with the complication rate is relatively low,and the clinical symptoms were improved.This method is worthy of clinical use in the treatment of displaced humeral supracondylar fractures in children.
