摘 要:目的探讨早期营养支持管理模式对重症脑卒中患者营养状态及并发症的影响。方法选取2015年5月—2016年4月收治重症脑卒中患者64例为对照组,给予常规护理和早期营养支持。选取2016年5月—2017年4月收治的重症脑卒中患者64例为观察组,接受以护士为主导的四位一体式早期营养支持管理模式。对比2组营养指标变化、并发症发生率、ICU住院时间及机械通气时间。结果干预后1个月,观察组血清白蛋白(Alb)、血清前清蛋白(PA)、血红蛋白(Hb)和静脉血中维生素C(VC)水平优于对照组(P<0.05)。观察组肺部感染和肠道感染发生率低于对照组(P<0.05);观察组ICU住院时间及机械通气时间短于对照组(P<0.01)。结论以护士为主导的四位一体式早期营养支持管理模式应用于重症脑卒中患者,可有效改善营养状态,对降低并发症发生风险、促进康复具有积极意义。Objective To investigate the early nutritional support management on nutritional status and complications in severe stroke patients. Methods Si^ cty-four cases of severe stroke pa-tients were selected as the control group between May 2015 and April 20 1 6, recei-ving convention-al nursing management. Another 64 patients were included as the observation group between May2016 and April 20 1 7,receiving nurse-led quaternity early nutritional support management. The nutrition index,incidence of complications,length of ICU stay and duration of ventilation wereobserved and compared between two groups. Results Patients in the observation group had ahigher level of serum albumin (Alb), prealbumin (PA) , hemoglobin (Hb) and vitamin C (VC) compaed witti those in the control group at 1 month after inter- vention. The incidence of pulmona-ry infection and intestinal infection were lower in the obser-vation group than those of controls (P 〈 0.05). Patients in the observation group had a shorter length of ICU stay and duration of ventila-tion compared with those in the control group (P 〈0. 01). Conclusion Nurse-led quaternityealy nutritional support may improve the nutritional status of severe stroke patients,so as prove the rehabilitation.
