经面罩吸入糖皮质激素联合脾氨肽治疗过敏性鼻炎-哮喘综合征患儿的临床疗效分析

摘　　要：目的探讨面罩吸入糖皮质激素联合脾氨肽对过敏性鼻炎-哮喘综合征(CARAS)的治疗效果,尽可能改善患者肺功能。方法选取2013年6月-2016年12月在该院治疗的CARAS患者166例,根据治疗方式的不同分为试验组和对照组,每组83例。试验组采用面罩吸入糖皮质激素联合脾氨肽方案进行治疗,对照组仅进行面罩吸入糖皮质激素方案进行治疗。比较两组患者哮喘控制测试、肺功能变化情况及过敏性鼻炎症状评分。结果治疗6个月及10个月试验组过敏性鼻炎症状评分明显低于对照组(P<0.05);治疗后试验组哮喘控制测试评分明显高于对照组(P<0.05);试验组患者气道总阻力、周边弹性阻力和呼吸总阻力均低于对照组(P<0.05);试验组用力呼出50%及75%肺活量的呼吸流速(FEF50、FEF75)、第一秒用力呼气量(FEV1)、最大呼气中期流量(MMEF75/25)均高于对照组(P<0.05)。结论面罩吸入糖皮质激素联合脾氨肽对过敏性鼻炎-哮喘综合征的治疗效果优于单纯采用糖皮质激素治疗方案,患者肺功能明显改善。Objective To explore the curative effect of mask glucocorticoid inhalation combined with spleen aminopeptide in treatment of children with combined allergic rhinitis and asthma syndrome（ CARAS）,improve pulmonary function of these children as much as possible. Methods A total of 166 patients with CARAS treated in the Third Hospital of Xingtai from June 2013 to December 2016 were selected and divided into experimental group and control group according to different treatment methods,83 children in each group. The children in experimental group were treated by mask glucocorticoid inhalation combined with spleen aminopeptide,and the children in control group were treated by mask glucocorticoid inhalation. The scores of asthma control test,pulmonary function changes,allergic rhinitis symptom scores in the two groups were compared. Results The scores of allergic rhinitis at 6 and 10 months after treatment in experimental group were statistically significantly lower than those in control group（ P0. 05）. After treatment,the score of asthma control test in experimental group was statistically significantly higher than that in control group（ P0. 05）. The total airway resistance,peripheral elastic resistance,and total respiratory resistance in experimental group were statistically significantly lower than those in control group（ P 0. 05）. FEF50,FEF75,FEV1,and MMEF75/25 in experimental group were statistically significantly higher than those in control group（ P 0. 05）.Conclusion The curative effect of mask glucocorticoid inhalation combined with spleen aminopeptide in treatment of children with CARAS is better than that of glucocorticoid therapy alone,and the pulmonary function of these children is improved significantly.
