中药联合输尿管镜钬激光碎石术治疗嵌顿性输尿管上段结石的临床观察

摘　　要：目的:探讨中药对输尿管镜钬激光碎石术治疗嵌顿性输尿管上段结石术后排石疗效。方法:将80例单侧嵌顿性输尿管上段结石患者随机分为两组,治疗组:40例,对照组:40例,两组均行输尿管镜钬激光碎石治疗,治疗组术后配合中药排石治疗。比较两组:术后1个月(C反应蛋白、降钙素原、肌酐、谷丙转氨酶、谷草转氨酶、血红蛋白、尿红细胞、尿白细胞)变化及术后1个月结石清除率。结果:两组患者一般资料差异无统计学意义(P>0.05),但体重指数上差异有统计学意义(P<0.05);手术1个月前后两组患者肌酐、谷丙转氨酶、谷草转氨酶、血红蛋白变化上差异无统计学意义(P>0.05);C反应蛋白、降钙素原、尿红细胞镜检、尿白细胞镜检上治疗组较对照组显著降低(P<0.05);术后1个月清石率治疗组97.5%(39/40)明显优于对照组90%(36/40)(P=0.028)。结论:输尿管镜钬激光碎石术后配合中药排石治疗可提高嵌顿性输尿管上段结石碎石术后排出率,降低术后机体炎性反应。Objective ： To investigate the effect of traditional Chinese medicine treatment of patients with impacted upper ureter- al calculi treated with ureteroscope holmium laser lithotripsy. Methods ： Eighty cases of impacted upper ureteral calculus were ran- domly divided into 2 groups ：40 cases in the treatment group and 40 cases in the control group. The 2 groups underwent uretero- scopic holmium laser lithotripsy. For the treatment group, after operation, patients were treated with traditional Chinese medicine. For the control group, patients were given nothing. We compared two groups C reaction protein, procalcitonin, creatinine, alanine aminotransferase, aspartate aminotransferase, hemoglobin, urine red blood cells, white blood cell changes 1 month after operation and the stone clearance rate. Results ：There was no significant difference between the two groups patients＇ general information （P 〉 0.05 ）, but there was significant difference on body mass index （P 〈 0. 05 ）. Two groups＇ serum creatinine, alanine aminotrans- ferase, aspartate aminotransferase and hemoglobin changes had no significant difference before and 1 month after surgery （ P 〉 0. 05 ）. C reactive protein and procalcitonin, urine red blood cell mirror check and urine white blood cell microscopic examination of the treatment group were significantly lower than those of the control group （P 〈 0.05）. The stone clearance rate 97.5% （39/ 40） in the treatment group was significantly better than the control group＇s 90% （36/40） （P = 0.028）. Conclusion：The uretero- scopic holmium laser lithotripsy with traditional Chinese medicine can improve removal rate of impacted upper ureteral calculi and reduce postoperative inflammatory reaction.
