心力衰竭伴胰岛素抵抗状态中西医研究进展

摘　　要：心力衰竭是各种心脏病的终末结局,病死率高,并呈逐年上升趋势。近年研究发现心力衰竭与胰岛素抵抗互为因果,共同推动疾病的进展,严重影响人类健康。明确心力衰竭伴胰岛素抵抗状态的病因机制,并寻求有效治疗方案是一项亟待攻克的医学难题。现代医学尚未形成整体统一的治疗原则,多采用抗心衰药物合并胰岛素增敏剂治疗心力衰竭伴胰岛素抵抗,但两类药物在一定程度上相互拮抗,不能协同起效,因而临床应用时存在一定局限性。在祖国医学的辨证体系中,心力衰竭与胰岛素抵抗统属于一个相同病机：气阴两虚、痰瘀互结,中医治疗在整体观念的指导下益气养阴、活血化痰,疗效确切。中西医治疗各有优势,较西医治疗,中医药具有全方位、多靶点的优势,并取得了显著成效。文章整理、分析近年来心力衰竭伴胰岛素抵抗的中西医研究进展,并综述于下。Heart failure is the end result of all kinds of heart disease,the mortality rate is high,and showed an upward trend year by year. In recent years,studies have found that heart failure and insulin resistance interact with each other to promote the progress of the disease,a serious impact on human health. To clarify the etiology and mechanism of heart failure with insulin resistance,and to seek an effective treatment is an urgent medical problem. Modern medicine has not yet formed a unified treatment principle,the use of anti heart failure drugs in treatment of heart failure with insulin resistance and insulin sensitizer,but two kinds of drugs to a certain extent antagonistic to each other,don＇t work,so there are some limitations in clinical application. In the traditional Chinese medicine syndrome differentiation system,heart failure and insulin resistance belongs to the same pathogenesis ：Qi and Yin deficiency,phlegm and blood stasis,Chinese medicine treatment in the overall concept under the guidance of supplementing Qi and nourishing Yin,promoting blood circulation and resolving phlegm,curative effect. Traditional Chinese and Western medicine treatment has advantages,compared with Western medicine treatment,traditional Chinese medicine has the advantages of omnibearing and multi target,and has achieved remarkable results. In this paper,we reviewed the recent advances in the research of traditional Chinese medicine and Western medicine in the treatment of heart failure with insulin resistance.
