牵引、中药外敷配合推拿治疗重度神经根型颈椎病的临床研究

摘　　要：目的观察牵引、中药外敷配合推拿治疗重度神经根型颈椎病的临床效果。方法 74例患者随机分为观察组与对照组各37例,对照组给予常规推拿治疗,观察组在对照组治疗的基础上给予牵引、中药外敷治疗,两组的治疗观察时间为1个月。结果观察组总有效率97.30%高于对照组的81.08%(P<0.05)。两组治疗前JOA评分差别不大(P>0.05)。治疗后两组JOA评分均高于治疗前(均P<0.05),且治疗后观察组JOA评分优于对照组(P<0.05)。两组治疗前全血黏度比较差别不大(P>0.05)。两组治疗后的全血高切黏度与全血低切黏度均低于治疗前(均P<0.05),同时治疗后观察组全血高切黏度与全血低切黏度均低于对照组(均P<0.05)。两组治疗前炎症因子表达比较差别不大(P>0.05)。两组治疗后的血清白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)值均低于治疗前(均P<0.05),治疗后观察组的血清IL-6和TNF-α值也均低于对照组(均P<0.05)。结论牵引、中药外敷配合推拿治疗重度神经根型颈椎病能促进改善颈椎功能,提高治疗效果,其作用机制可能与抑制炎症因子表达和改善血液流变学有关。Objective： To explore the clinical effects of traction,external application of TCM and massage on severe cervicalspondylotic myelopathy. Methods： The retrospective study method was used in 74 patients. According to the different treatment methods,they were divided into the observation group and the control group,37 cases in each. The control group was given conventional massage therapy ,while the observation group was treated with traction,external application of TCM based on the treatment of control group. The two groups were treated for a period of 1 month. Results： The total effective rate of the observation group and the control group were 97.30% and 81.08%,respectively;the total effective rate of the observation group was significantly higher than that of the control group （P〈 0.05）. There was not much difference in JOA scores between the two groups before treatment （P〉 0.05）. After treatment,JOA scores of the two groups were higher than those before the treatment （P〈 0.05）. JOA score of the observation group was significantly better than that of the control group （P〈 0.05）. There was little dif- ference in whole blood viscosity between the two groups before treatment （P〉 0.05）. After treatment,whole blood viscosity and blood viscosity of the two groups were lower than that before treatment （P〈0.05）. At the same time, after treatment,whole blood viscosity and blood viscosity of the observation group were lower than those of the control group（P〈 0.05 ）. Before treatment,there was little difference in the expression of inflammatory factors in the two groups（P〉 0.05）. The values of serum IL-6 and TNF-α in the two groups were lower than those before treatment （P 〈 0.05）. After treatment,the values of serum IL-6 and TNF-α in the observation group were also lower than those of the control group（P〈0.05）.
