吴氏手法配合中药热敷及功能锻炼改善粘连前期肩周炎疼痛的临床研究

摘　　要：目的观察吴氏手法配合中药热敷及功能锻炼治疗粘连前期肩周炎疼痛的临床疗效。方法将患者69例按随机数字表法分为A组35例和B组34例。A组采用吴氏手法配合中药热敷及功能锻炼;B组采用双氯芬酸钠缓释片口服加双氯芬酸二乙酰乳胶剂外用。治疗2周。观察两组治疗后1,2,4,12周肩关节疼痛及压痛改善情况。结果 A组疼痛评分在治疗后1周改善显著(P<0.05),B组疼痛评分在治疗后2周改善显著(P<0.05),组间比较在各访视点两组疼痛评分差异均有统计学意义(P<0.05)。两组治疗后肩部压痛均有改善(P<0.05);治疗后2周、4周以及12周组间差异有统计学意义(P<0.05)。结论吴氏手法配合中药热敷及功能锻炼治疗粘连前期肩周炎疼痛起效快,疗效佳。Objective： To observe the clinical efficacy of Wu＇s manipulation combined with TCM hot compress and dirigation on the pain of scapulohumeral periarthritis in early stage of Adhesion. Methods： 69 patients were randomly divided into A group of 35 cases and B group of 34 cases. Group A was treated by Wu＇s manipulation combined with TCM hot comPress and dirigation ;Group B was treated with diclofenac sodium sustained-release tablets plus diclofenac diacetyl latex for external use. After 2 weeks of treatment,the improvement of shoulder pain and tenderness on the l st,2nd,4th and 12th weeks of treatment were observed. Results： The score of pain in group A was statistically significant at 1st week after treatment （P〈 0.05）. The score of pain in group B was statistically significant at the second week after treatment（P 〈 0.05）. There were significant differences in scores of pain between the two groups （P 〈 0.05）. There was no significant difference in shoulder tenderness between the two groups before treatment and 1 week after treatment （P〉 0.05）. There was significant difference in shoulder tenderness between the two groups at 2 weeks,4 weeks and 12 weeks after treatment （P〈 0.05）. Conclusion： Wu＇ s manipulation combined with TCM hot compress and dirigation on the pain of scapulohumeral periarthritis in early stage of adhesion has a faster and better effect.
