摘  要:目的探究内皮型一氧化氮合酶(e NOS)基因第4内含子基因多态性与冠状动脉扩张症(CAE)的关系。方法纳入71例住院患者,采用病例对照研究方法选择冠状动脉扩张组30例和冠状动脉正常组41例。收集患者年龄、性别、吸烟史、饮酒史等一般资料。采用聚合酶链反应(PCR)技术,分析比较两组患者一氧化氮合酶基因第4内含子基因多态性。结果冠状动脉扩张组患者病变累及单支冠状动脉最多,共14例(46.7%);累及2支、3支冠状动脉患者分别为8例(26.7%)、8例(26.7%)。病变累及右冠状动脉最多,占44.4%;累及前降支、回旋支分别占31.5%和24.1%。冠状动脉扩张组与冠状动脉正常组aa、ab、bb基因型分布频率分别为13.3%、33.3%、53.3%和4.9%、17.1%、78%(P>0.05)。冠状动脉扩张组与冠状动脉正常组a等位基因分布频率为30%、13.4%(P<0.05)。经Logistic回归分析显示,内皮型一氧化氮合酶基因第4内含子a等位基因是冠状动脉扩张症发生的独立危险因素(P<0.05,OR=3.327,95%CI=1.083~10.226)。结论内皮型一氧化氮合酶基因第4内因子a等位基因可能是冠状动脉扩张症发病的独立危险因素。Aim To investigate the relationship between intron 4 a/b gene polymorphisms of endothelial nitric oxide synthase(e NOS) gene and coronary artery ectasia(CAE). Methods The research was performed by case-control study. The CAE group included 30 patients with coronary artery ectasia on coronary angiogram. The control group contained 41 patients with normal coronary artery. The study collected the data of patients' gender,age,smoking history,drinking history,ect. Polymorphism chain reaction(PCR) technique was used to identify the e NOS intron 4 a/b gene polymorphisms. Results The lesion involved single coronary artery in most cases was 46.7%(14 cases),the lesions involved in two and three vessels were 26. 7%(8 cases) and 26. 7%(8 cases),respectively. The right coronary artery(RCA) was the most frequently involved vessel(44.4%),the left anterior descending artery(LAD) and the left circumflex artery(LCX) were 31. 5% and 24. 1%,respectively. The frequencies of e NOS gene phenotypes in CAE group and control group for "aa","ab","bb"were 13.3%,33.3%,53.3% and 4.9%,17.1%,78%(P〈0.05),respectively.The presence of"a"type allele of e NOS gene in CAE group and control group were 30% and 13.4%(P〈0.05),respectively. Logistic regression analysis showed that "a"type allele of e NOS gene was an independent risk factor for CAE(P〈0.05,OR = 3.327,95% CI = 1.083 - 10.226). Conclusion The"a"type allele of e NOS gene may be an independent risk factor for the occurrence of CAE.
