7例横纹肌样脑膜瘤临床特征与MR表现分析

摘　　要：目的探讨横纹肌样脑膜瘤(RM)的MRI特征性表现及其临床特点。方法对7例(男3例,女4例)患者的临床资料及MRI表现进行回顾性分析,所有患者均经手术病理证实为RM。结果 7例RM中,2例位于鞍旁,1例位于额叶,1例位于颞叶,1例位于枕叶,1例位于左侧额部颅板内外,1例位于右侧侧脑室。7例RM中,6例为实性,1例为囊实性。6例实性病灶中,3例呈等T1、等T2信号,2例呈稍长T1、稍长T2信号,1例呈长T1、长T2信号;1例囊实性病变,实性部分呈等T1、等T2信号,囊性部分呈长T1、长T2信号。6例实性病灶DWI(b=1000 s/mm2)1例呈低信号,1例呈等信号,3例呈稍高信号,1例呈明显高信号;1例囊实性病灶DWI(b=1000 s/mm2)囊性部分呈低信号,实性部分呈稍高信号。1例实性病灶可见轻度瘤周水肿(水肿范围<1 cm),1例囊实性病灶周围见轻度瘤周水肿,余均未见瘤周水肿。1例实性病灶可见局部骨质肥厚并骨质破坏。增强扫描2例实性病灶呈明显均匀强化;余4例实性病灶呈不均匀强化,中心可见相对低信号未强化区;1例囊实性病变囊性部分未见明显强化,实性部分呈明显不均匀强化。结论 RM的MR表现具有一定特征性,可伴有瘤周水肿,可发生囊变、坏死,也可伴有骨质破坏。Objective To describe and characterize clinical features and imaging findings associated with rhabdoid meningioma（ RM）. Methods MRI appearances and pathological types of rhabdoid meningiomas in 7 cases（ 3 men and 4 women） were retrospectively analysed. Results In 7 cases with RMs,2 cases were located in the parasellar region,1 in the frontal lobe,1 in the temporal lobe,1 in the occipital lobe,1 inside and outside the left frontal skull and 1 in the right lateral ventricle. In 7 cases of RMs,6 cases were solid,1 was solid-cystic. Of the 6 solid lesions,3 cases showed iso-intensity on T1 WI and T2 WI. 2 cases showed slightly long T1,slightly long T2 signal intensity（ SI）. 1 case showed long T1,long T2 SI. The solid-cystic lesion＇s SI is complicated,the solid area showed isointensity on T1 and isointensity on T2 SI,while the cystic area showed long T1 and long T2 SI. Peritumoral edema were seen in 2 lesions. 1 lesion had hyperostosis and bone destruction. On gadolinium-enhanced T1 WI,homogeneous enhancement was seen in 2 lesions,and heterogeneous enhancement was seen in 4 lesions. Conclusion MR features of RMs have certain characteristics. They tend to have peritumoral edema,cystic components,and bone involvement.
