急性胃扭转的CT征象分析

摘　　要：目的探讨急性胃扭转的CT影像特点。方法回顾性分析经手术证实的12例急性胃扭转患者资料,术前均行CT扫描,观察急性胃扭转的CT征象,总结其影像特点。结果 CT检查示食管裂孔疝9例(75%);12例(100%)急性胃扭转患者的CT图像均显示胃窦与幽门移行区存在,且移行区均无占位或其他梗阻因素;胃窦位于胸腔内有9例(75%),胃体位于胸腔内有3例(25%),胃底位于胸腔内有5例(41.7%);12例急性胃扭转患者胃窦位置高于或等于胃底,其中7例(58.3%)胃窦位置高于胃底,5例(41.7%)胃窦位置与胃底等高。结论急性胃扭转在CT图像上有一定特点,胃窦与幽门移行区存在,胃窦位置高于或等于胃底,CT检查是诊断急性胃扭转的重要方法。Objective To assess the diagnostic performance of CT signs of acute gastric volvulus. Methods Retrospectively reviewed CT findings in 12 patients with surgically confirmed acute gastric volvulus,to evaluate CT images for acute gastric volvulus. Results CT examination in 9 cases（ 75%） of esophageal hiatal hernia; 12 cases（ 100%） of acute gastric volvulus the CT images of patients showed an antropyloric transition point without any abnormality at the transition zone; Intrathoracic position of the antrum in 9 cases（ 75%）,Intrathoracic position of the body in 3 cases（ 25%）,Intrathoracic position of the fundus in 5 cases（ 41. 7%）; The antrum at the same level or higher than the fundus,Among them 7 cases（ 58. 3%） of gastric antrum position higher than the fundus,5 cases（ 41. 7%） of gastric antrum and the fundus at the same level. Conclusion There are some characteristics of acute gastric volvulus on CT image,the presence of a gastropyloric transition zone and the antrum at the same level or higher than the fundus,CT examination is the important method in the diagnosis of acute gastric volvulus.
