针刺联合补肾健骨汤治疗腰椎骨质增生性腰痛及对DDK-1、硬骨素水平的影响

摘　　要：目的:观察针刺联合补肾健骨汤治疗腰椎骨质增生性腰痛疗效及对DDK-1、硬骨素水平的影响。方法:选取确诊的腰椎骨质增生继发性腰痛患者110例,随机分为两组各55例,对照组在牵引基础上给予塞来昔布;治疗组在牵引基础上加用穴位针刺联合补肾健骨汤辅助治疗。比较两组近期疗效、简化McGill疼痛量表评分。结果:治疗组近期疗效显著优于对照组(P<0.05);治疗组治疗后中医证候积分、PRI、VAS及PPI评分均显著低于对照组(P<0.05);治疗组治疗后Oswestry功能障碍指数问卷评分和HRQL评分显著优于对照组(P<0.05);治疗组治疗后DKK-1和硬骨素水平显著高于对照组(P<0.05);两组药物不良反应发生率比较无显著性差异(P>0.05)。结论:中西医结合疗法治疗腰椎骨质增生性腰痛临床疗效显著优于单用西药。Objective: to approach the effect of acupuncture with Bushen Jiangu decoction on lumbago due to lumbar vertebrae hyperosteogeny and DDK-1,sclerostin level. Methods: 110 cases of selected patients with secondary lumbago due to hyperosteogeny were divided randomly into two groups,55 cases in each group. The control group was treated with Celecoxib on the basis of traction,while the treatment group was treated plus with acupuncture acupoint combined with Bushen Jiangu decoction on the basis of control group. The short-term effect and the simplified McGill pain scale score were compared in two groups. Results: The short-term effect of treatment group was significantly better than that of control group( P < 0. 05). After treatment: TCM syndrome score,PRI,VAS and PPI scores in treatment group were significantly lower than in control group and before treatment( P <0. 05); and the questionnaire score of Oswestry disability indexes and the HRQL score in treatment group were significantly better than in control group and before treatment( P < 0. 05); DDK-1 and sclerostin levels in treatment group were significantly higher than in control group and before treatment( P < 0. 05). Compared the incidence of adverse drug reactions betweem two groups,there was not significant difference( P > 0. 05). Conclusion: The clinical effect of acupuncture combined with Bushen Jiangu decoction on lumbago due to lumbar vertebrae hyperosteogeny is significantly better than single western medicine.
