保心安胶囊联合曲美他嗪片对2型糖尿病合并心力衰竭患者pro-BNP的影响研究

摘　　要：目的观察保心安胶囊联合曲美他嗪片对2型糖尿病合并慢性心力衰竭(CHF)患者的临床疗效及治疗前后心功能及空腹血糖、血浆B型脑利钠肽前体(pro-BNP)和超敏C-反应蛋白(hs-CRP)的影响。方法将符合纳入标准的82例患者随机分为对照组(n=40)和治疗组(n=42)。对照组给予西医常规治疗,加用曲美他嗪片。治疗组在对照组基础上加以保心安胶囊服用,共治疗6个月,观察2组治疗前后的心功能情况[心输出量(CO)、每搏输出量(SV)、左心室射血分数(LVEF)、左心室舒张晚期与早期血流峰值速度比值(E/A)]并检测空腹血糖(FPG)、血清pro-BNP和hs-CRP水平及超声心动图检测心功能。结果 2组治疗后FPG、pro-BNP、hs-CRP较治疗前降低,治疗组心功能改善程度和FPG、pro-BNP、Hs-CRP降低程度均优于对照组差异均有统计学意义(P<0.05)。治疗后2组E/A、LVEF、CO及SV均较本组治疗前提高,且治疗组较对照组提高的更明显,差异均有统计学意义(P<0.05)。结论保心安胶囊联合曲美他嗪片可抑制DM合并CHF患者的非特异性炎性反应,更好的控制血糖,改善心功能,从而提高临床疗效。Objective To observe the clinical efficacy of Baoxin＇an capsule combined with trimetazidine tablets on type 2 diabetes mellitus（ T2 DM） complicated by chronic heart failure（ CHF） and their effects on heart function,fasting blood glucose,pro-brain natriuretic peptide（ pro-BNP） and high sensitivity C-reactive protein（ Hs-CRP）. Methods Eighty-two patients with T2 DM complicated by CHF were randomly divided into control group（ n = 40） and observation group（ n = 40）.The patients in control group were treated by conventional western medicines plus trimetazidine tablets,however,the patients in observation group,on the basis of control group,were treated by Baoxing＇an capsule,with a treatment curse of 6 months for both groups. The cardiac function indexes including cardiac output（ CO）,stroke volume（ SV）,left ventricular ejection fraction（ LVEF）,ratio of E/A before and after treatment were observed and compared between two groups. Moreover the levels of fasting blood glucose（ FPG）,serum pro-BNP,hs-CRP were detected,in addition,the cardiac function was examined by ultrasound cardiography. Results After treatment the levels of FPG,pro-BNP and Hs-CRP were decreased in both groups,as compared with those before treatment,moreover the improvement degree of cardiac function and decrease degree of FPG,pro-BNP and hs-CRP in observation group were superior to those in control group（ P〈0. 01）. After treatment,the ratio of E/A,LVEF,CO and SV were significantly increased in both groups,as compared with those before tratment,moreover,the increase degree in observation group was more obvious than that in control group（ P〈0. 05）. Conclusion Baoxin ＇an capsule combined with trimetazidine tablets can inhibit the non-specific inflammatory reaction of patients with T2 DM complicated by CHF,which can effectively control blood glucose,improve heart function so as to enhance clinical curative effects.
