超声动态监测联合中医结合治疗ASO疗效及对双下肢峰值血流速度的影响

摘　　要：目的探讨超声动态监测联合中医结合治疗下肢动脉硬化闭塞症(ASO)的研究及对双下肢峰值血流速度的影响。方法 ASO患者90例,随机分为对照组和研究组,每组45例。对照组应用西洛他唑治疗,研究组在对照组基础上采取超声动态监测联合中医治疗。观察2组治疗前后动力学指标(峰值血流速度、血流搏动指数、阻力指数)、甲襞微循环积分、踝肱指数(ABI)、炎性因子[肿瘤坏死因子(TNF-α)、C-反应蛋白(CRP)、白介素-6(IL-6)、细胞间黏附分子-1(ICAM-1)]。结果治疗前2组患者超声检测下峰值血流速度、阻力指数以及血流波动指数差异无统计学意义(P>0.05),治疗后均得到一定改善,研究组显著优于对照组(P<0.05);治疗前2组患者甲襞微循环积分以及ABI无明显区别(P>0.05),治疗后均得到一定改善,研究组显著优于对照组(P<0.05);治疗前2组患者CRP、TNF-α、IL-6、ICA-1无明显区别(P>0.05),治疗后均得到一定改善,研究组显著优于对照组(P<0.05)。结论 ASO患者在超声动态检测下联合应用中医治疗,有利于提高治疗效果,改善患者双下肢峰值血流速度。Objective To investigate the effects of ultrasonic dynamic monitoring combined with traditional Chinese medicine on the peak blood flow velocity of both lower extremities in treatment of arteriosclerosis obliterans（ SAO）. Methods Ninety patients with ASO who were admitted and treated in our hospital from January 2016 to December 2016 were randomly divided into control group（ n = 45） and observation group（ n = 45）. The patients in control group were treated by oral cilostazol,however,the patients in observation group,on the basis of control group,were treated by ultrasonic dynamic monitoring combined with traditional Chinese medicine. The haemodynamics indexes including peak blood flow velocity,flow velocity pulsation index and resistance index before and after treatment,nail wall microcirculation integral,ankle/brachial index（ ABI）,inflammatory factors including tumor necrosis factor（ TNF-α）,C-reactive protein（ CRP）,interleukin-6（ IL-6）,intercellular adhesion molecule-1（ ICAM-1） were detected and compared between two groups. Results Before treatment there were no significant differences in peak blood flow velocity,resistance index and fluctuation index of blood flow between two groups（ P〉0. 05）,however,after treatment which in observation group were superior to those in control group（ P〈0. 05）. Before treatment there were no significant differences in nail wall microcirculation integral and ABI between two groups（ P〉0. 05）,however,after treatment which in observation group were superior to those in control group（ P〈0. 05）.Before treatment there were no significant differences in CRP,TNF-α,IL-6,ICA-1 between two groups（ P〉0. 05）,however,after treatment which in observation group were superior to those in control group（ P〈0. 05）. Conclusion The ultrasonic dynamic monitoring combined with traditional Chinese medicine can enhance therapeutic effects on ASD,which can improve the peak blood flow velocity of both lower extremities of patients with ASO
