瘤型麻风患者联合化疗后复发误诊为扁平苔藓1例

摘　　要：患者男,50岁。原患麻风病经治疗后临床痊愈。近期全身泛发丘疹、结节,伴微痒6个月。患者无麻风病家族史。查体:面部、手背和下肢伸侧见多发散在、大小不等的皮色或褐红色丘疹、结节,边界清楚。临床初步误诊为扁平苔藓。皮损组织病理:表皮萎缩,真皮层见程度不一泡沫化的巨噬细胞以及少量淋巴细胞浸润。抗酸染色见大量抗酸杆菌。最后诊断为瘤型麻风患者复发。A 50-year-old male patient presented with multiple papules, nodules involving the whole body accompanied with slight pruritus for 6 months without family history of leprosy. Dermatological examination showed a number of isolated and variable sizes of light red to dark red papules and nodules on the face, the back of the hand and the extensor aspect of lower limbs. He was clinically misdiagnosed as lichen planus. Histopathology examination demonstrated epidermal atrophy, different degrees of foamy macrophages and a few lymphocytes in the dermis layer. Acid-fast staining showed a large number of acid-fast bacilli. Final diagnosis was recurrence of lepromatous leprosy.
