摘 要:目的通过采集2014年1月—2016年10月济宁地区的疟原虫阳性诊断数据,对RDT胶体金法在疟疾低传播区的应用进行评估。方法本研究包括济宁地区272名输入性发热的疑似患者数据。采用广州万孚疟疾快速诊断试剂(胶体金法),进行快速而简单的检测,然后以PCR结果为标准,并与显微镜镜检结果进行比较分析。结果 272份血样采用镜检法、RDT胶体金法和PCR检测疟疾感染,阳性率分别为13.6%、15.1%和14.3%,三种检测方法阳性率差异无统计学意义(χ~2=0.89,P>0.05);以PCR为标准,镜检法漏诊率5.1%,误诊率5.1%,灵敏度94.9%,特异性100.0%,阳性预测值100.0%,阴性预测值99.1%。RDT胶体金法漏诊率0,误诊率4.9%,灵敏度97.4%,特异性98.7%,阳性预测值92.7%,阴性预测值99.6%。结论在疟疾低发病和低传播地区,疟原虫RDT胶体金法在检测输入性疟疾感染较显微镜镜检有更高的灵敏度和阴性预测值,可作为显微镜检查的辅助工具,适于在基层大规模的推广应用。Objective To evaluate the application of the Wondfo malaria RDT colloidal gold method in the low transmission zone of malaria in Jining area from January 2014 to October 2016. Methods There were 272 imported suspected malaria patients with fever, and their blood samples were detected with the Wondfo malaria RDT colloidal gold method, PCR(its results as the standard), and microscopy. Results The positive rates of the microscopy, RDT colloidal gold method, and PCR were 13.6%, 15.1% and 14.3% respectively, and there was no significant difference among them(χ~2= 0.89, P〈0.05). With PCR as the standard, the missed diagnosis rate of the microscopy was 5.1%, the misdiagnosis rate was 5.1%, the sensitivity was 94.9%, the specificity was 100.0%, the positive predictive value was 100.0%, and the negative predictive value was 99.1%; the missed diagnosis rate of the RDT colloidal gold method was 0, the misdiagnosis rate was 4.9%, the sensitivity was 97.4%, thespecificity was 98.7%, the positive predictive value was 92.7%, and the negative predictive value was 99.6%. Conclusion In the low-incidence and low-transmission areas of malaria, the malaria RDT colloidal gold method has the higher sensitivity and negative predictive value compared with the microscopy in the detection of imported Plasmodium infection, and therefore, it can be used as an auxiliary tool of microscopic examinations at the grass-root level.
