盐酸达克罗宁胶浆与盐酸奥布卡因凝胶用于无痛人工流产术的比较

摘　　要：目的比较盐酸达克罗宁胶浆与盐酸奥布卡因凝胶复合异丙酚在人工流产术中的效果。方法选择人工流产术的受术者60例，随机分为达克罗宁组和奥布卡因组，每组30例，两组均静脉注射异丙酚后分别行盐酸奥布卡因凝胶和盐酸达克罗宁胶浆宫颈涂抹和宫颈管注射，记录HR、BP、RR、SpO2，比较术中体动反应、宫颈松弛度、异丙酚用量、术中出血量及苏醒时间。结果两组患者麻醉前和麻醉中收缩压、舒张压、心率及SpO2比较差异均无统计学意义（P〉0．05）。达克罗宁组受术者体动反应例数、宫颈松弛度、异丙酚用量及苏醒时间均低于奥布卡因组，差异均有统计学意义（P〈0，05），两组术中出血量差异无统计学意义（P〉0．05）。结论盐酸达克罗宁胶浆局部应用复合异丙酚比盐酸奥布卡因凝胶复合异丙酚在人工流产术中具有镇痛效果好及异丙酚用量少的优点。Objective To compare the efficacy of dyclonine hydrochloride mortar and oxybuprocaine hydrochloride gel combined with intravenous propofol in painless artificial abortion. Methods Sixty subjects accepting painless artificial abortion were randomly divided into dyclonine group and oxybuprocaine group, 30 cases in each group. The subjects were intravenously injected with propofol to fall asleep, then dyclonine hydro- chloride mortar and oxybuprocaine hydrochloride gel were respectively applied to the cervical smear and the cervical tube injection after disappearance of eyelash reaction. Artificiai abortion was performed after 1 min. HR, BP, RR, Sp02, physical reaction, cervical relaxation, propofol dosage, intraoperative amount of bleeding and awaking time were recorded and compared in the two groups. Results There were no significant differ- ence in blood pressure, heart rate and SpO~ between the two groups before and during the anesthesia （P〉O.05）. The cases of motor responses, cervical relaxation, propofol dosage and awaking time were lower in dyclonine group than those in oxybuprocaine group （P〈0.05）. There was no significant difference in intraoperative amount of bleeding between the two groups （P〉0.05）. Conclusion Dyclonine hydrochloride mortar applied locally combined with intravenous propofol is of better analgesic effect and less propofol dosage compared with oxybu- procaine hydrochloride gel combined with intravenous propofol in painless artificial abortion.
