摘 要:目的探讨三种不同三维扫描质量对输卵管超声造影准确性的影响。方法将140例因不孕症准备行腹腔镜检查的患者随机分为3组,分别采用低扫描质量、中扫描质量和高扫描质量,获取输卵管三维超声造影图像,以腹腔镜检查结果为金标准,评价不同三维扫描质量对三维超声造影准确性的影响。结果 140例患者共获得247条满意的输卵管造影图像,低扫描质量组灵敏度为77.4%、特异度为88.0%,中扫描质量组灵敏度为89.5%、特异度为82.6%,高扫描质量组灵敏度为54.5%、特异度为78.0%;三组间无统计学差异。低扫描质量组约登指数为0.65,Kappa值是0.658;中扫描质量组约登指数为0.72,Kappa值是0.724;高扫描质量组约登指数为0.33,Kappa值是0.329;三组检查结果和腹腔镜检查结果的符合率分别是83.9%、86.4%和69.8%;中扫描质量组的符合率、灵敏度、约登指数和Kappa值高于其他两组。结论不同的扫描质量对输卵管通畅性判断有一定的影响,中等扫描质量参数准确性优于其他两组。Objective To identify the influences of scanning quality with three different volumes on the accuracy of fallopian tube patency in hysterosalpingo-contrast sonography. Methods 140 patients with infertility for fallopian tubes who prepared to undergo laparoscopy were divided into three groups. The scanning quality with the low volume, the mid volume and the high volume was applied to acquire three-dimensional ultrasound contrast images of fallopian tubes. Laparoscopy results were the golden standards, which were used to evaluate influences of scanning quality with different volumes on three-dimensional ultrasound contract accuracy of fallopian tubes. Results 140 patients had a total of 247 fallopian tubes to acquire satisfactory angiography images. The sensitivity in the low, mid and high groups was 77.4G, 89.5% and 54.5% respectively ; the specificity was 88.0, 82.6G and 78.0% respectively. There was no obvious statistic difference between scanning inspection results in three groups. Youden index in the low, mid and high groups was 0.65, 0.72 and 0.33, Kappa in the low, mid and high groups was 0. 658, 0. 724 and 0. 329, respectively. The coincidence rates of the three groups between hysterosalpingo-contrast sonography and of laparoscopy were 83.9% ,86.4% and 69.8% respectively. The coincidence rate, sensitivity , Youden index, Kappa value in the mid group was higher than the other groups. Conclusions The different scanning quality had certain effect on judging fallopian tube patency. The accuracy of mid scanning quality was superior to other two groups.
