摘 要:目的分析1 100例妇科阴道分泌物检查结果。方法选取2015年3月至2016年12月于福建中医药大学附属南平人民医院就诊的女性患者1 100例,对所有患者进行白带分泌物标本涂片以及显微镜检查。并分析对比不同年龄、不同月份以及不同清洁度妇女的阴道感染情况。同时,对所有患者分泌物标本进行常规的盐水检测以及五项联合测定试剂法检测,分别对比两种检查结果中患者细菌性阴道病阳性率。结果 35~54岁妇女阴道感染发生率为53.21%(298/560),高于20~34岁的40.00%(144/360)以及≥55岁的29.44%(53/180),而20~34岁又高于≥55岁的感染发生率,差异有统计学意义(均P<0.05)。7~9月份妇女阴道感染发生率为61.03%(260/426),高于1~3月份、4~6月份、10~12月份的24.83%(37/149)、39.22%(100/255)、35.56%(96/270),差异有统计学意义(均P<0.05)。清洁度Ⅰ~Ⅱ的真菌、滴虫感染率分别为14.44%(78/540)、3.89%(21/540),低于Ⅲ~Ⅳ的40.00%(224/560)、27.14%(152/560),差异有统计学意义(均P<0.05)。常规盐水检测细菌性阴道病的阳性率为31.36%(345/1100),低于五项联合测定试剂法的43.82%(482/1100),差异有统计学意义(P<0.05)。结论不同年龄段、不同季节阴道感染的情况,以及不同清洁度真菌感染、滴虫感染的情况通过采用常规盐水涂片法检测及五项联合测定试剂法检测对比,发现五项联合测定试剂法诊断细菌性阴道病的阳性率显著高于常规盐水检测,具有较高的临床诊断价值,值得推广应用。Objective To analyze the gynecological examination results of 1,100 cases of vaginal secretion.Methods 1,100 female patients in our hospital from March 2015 to December 2016 were selected;microscopic examination was performed on the smears of their vaginal secretion.Incidences of vaginal infections were analyzed and compared between different ages,different months and different vaginal cleanliness.Conventional detection with saline and ELISA kit were used respectively to detect all the specimens,and the positive rates of bacterial vaginosis by the two methods were compared.Results The rate of vaginal infections was 53.21%(298/560)in 35-54 years group,40.00%(144/360)in 20-34 years group,and 29.44%(53/180)in≥ 55 years group,with significant differences(Ps0.05).The incidence rate of vaginal infections was 61.03%(260/426)in Jul-Sep,24.83%(37/149)in Jan-Mar,39.22%(100/255)in Apr-Jun,and 35.56%(96/270)in Oct-Dec,with significant differences(Ps0.05).The rates of fungal infection and trichomonas infection were 14.44%(78/540)and 3.89%(21/540)respectively in I-II cleanliness group,lower than the 40.00%(224/560)and 27.14%(152/560)in the III-IV cleanliness group,with significant difference(P0.05).The positive rate of bacterial vaginosis was 31.36%(345/1100)by conventional detection with saline,lower than the 43.82%(482/1100)by ELISA kit,with significant difference(P0.05).Conclusion The incidence of vaginal infections varied in different age group and different season.The positive rate of bacterial vaginosis by ELISA kit detection was significantly higher than that by conventional detection with saline,thus has high clinical diagnostic value.
