摘　　要：目的探讨出院计划服务模式应用于上尿路结石手术患者的效果。方法将上尿路结石手术患者130例随机分为观察组和对照组各65例。对照组采用常规围手术期护理和电话随访,观察组实施出院计划服务模式,包括患者评估、计划制定、计划实施与效果评价。结果出院时观察组结石防治与双J管留置并发症预防知识知晓率、患者满意度显著高于对照组,延迟拔管发生率及出院后6个月双J管脱管发生率显著低于对照组(P<0.05,P<0.01)。结论出院计划服务模式用于上尿路结石手术患者,有利于提高患者相关知识知晓率,降低双J管留置并发症及延迟拔管发生率,提高患者满意度。Objective To explore the effect of discharge planning service model on patients undergoing upper urinary tract calculi surgery. Methods A total of 130 patients undergoing upper urinary tract calculi surgery were randomized into an observation group （n = 65） and a control group （n= 65）. The control group received routine perioperative nursing and telephone follow-up, while the observation group was cared using discharge planning service model, which included patient assessment, plan making, plan imple- mentation and effect evaluation. Results At discharge, the awareness rate of knowledge on calculi prevention and treatment and complications prevention after retention of double J stent in the observation group were significantly higher than those of the control group, patient satisfaction was significantly higher than that of the control group, and the rate of delayed withdrawal of double J stent and complications induced by retention of double J stent 6 months after discharge were significantly lower than those of the control group （P〈0.05, P〈0.01）. Conclusion Application of discharge planning service model in patients undergoing upper urina- ry tract calculi surgery, is conducive to enhance patients＇ awareness rate of related knowledge, reduce the rate of delayed withdrawal of double J stent and complications induced by retention of the stent, and improve patient satisfaction.
