CysC在老年患者术后辅助化疗肾功能监测中的运用

摘　　要：目的:研究Cys C在老年患者术后辅助化疗肾功能监测中的运用。方法:收集我院2013年1月~2015年1月的行化疗的恶性肿瘤老年患者80例,将其设置为观察组,另外71例的健康体检者则设置为对照组,观察血清胱抑素-C对于恶性肿瘤老年患者术后辅助化疗肾功能损害的监测作用。结果:两组化疗前的Cys C、Scr、BUN、eGFR的表达水平比较差异无统计学意义(P>0.05)。观察组患者化疗后2周期的Cys C、eGFR的表达水平高于对照组的,经统计学分析差异具有统计学意义(P<0.05)。含铂类化疗2周期的Cys C较化疗前有所升高,且经统计学分析差异具有统计学意义(P<0.05)。含铂类化疗2周期的eGFR较化疗前有所降低,且经统计学分析差异具有统计学意义(P<0.05)。非含铂类化疗2周期的Cys C较化疗前有所升高,且经统计学分析差异具有统计学意义(P<0.05)。非含铂类化疗2周期的eGFR较化疗前有所降低,且经统计学分析差异具有统计学意义(P<0.05)。含铂类药物化疗组化疗后2周期的Cys C量高于非含铂类药物化疗组化疗后2周期,且经统计学分析差异具有统计学意义(P<0.05)。两组患者Cys C、Scr、BUN与eGFR具有相关性,且Cys C与eGFR的相关性更为显著。对照组的Cys C、Scr、BUN与eGFR之间具有相关性(r=-0.77、-0.39、-0.26),其中Cys C与eGFR高度相关,显著强于后两者;观察组的Cys C、Scr、BUN与eGFR也具有相关性(r=-0.81、-0.47、-0.24),且Cys C与eGFR最为高度相关。结论:血清胱抑素-C能更为敏感的监测到恶性肿瘤老年患者术后辅助化疗的肾功能损害。Objective： To study the application of Cys C in the monitoring of renal function in elderly patients with postoperative adjuvant chemotherapy.Methods： 80 patients with malignant tumor in our hospital from January 2013-2015 January period were set as the observation group,another 71 cases of healthy subjects were set as control group,to observe the monitoring effect of serum cystatin-C for malignant tumor in elderly patients with postoperative adjuvant chemotherapy of renal impairment.Results： There was no significant expression levels in the two groups before chemotherapy Cys C,Scr,BUN,eGFR difference（ P〉0.05）.Patients in the observation group after chemotherapy 2 cycles of Cys C,the expression level of eGFR was higher than that in the control group,the statistical analysis showed significant difference（ P〈0.05）.Platinum containing chemotherapy for 2 cycles of Cys C than before chemotherapy was increased,and the statistical analysis showed significant difference（ P〈0.05）.Platinum containing chemotherapy for 2 cycles of eGFR than before treatment decreased,and the statistical analysis showed significant difference（ P〈0.05）.Non platinum based chemotherapy for 2 cycles of Cys C than before chemotherapy was increased,and the statistical analysis showed significant difference（ P〈0.05）.Non platinum based chemotherapy for 2 cycles of eGFR than before treatment decreased,and the statistical analysis showed significant difference（ P〈0.05）.Two groups of patients with Cys C,Scr,BUN and eGFR were correlated,and the correlation between Cys C and eGFR was more significant.Conclusion： Serum cystatin-C can be more sensitive to the elderly patients with malignant tumor operation monitoring of renal injury after chemotherapy.
