公共管理视角下公立医院公益性的内含及相关问题分析

摘　　要：公立医院的公益性问题是长久以来争论不休的话题。本文从公共管理的角度对公立医院的公益性进行界定,认为公立医院的公益性是指医疗产品和服务带来的正外部性。公立医院公益性弱化的主要原因不是政府财政投入不足,而是对于最能体现公益性的公共医疗卫生服务领域投入不够,而且允许公立医院合理合法地逐利。加强公立医院公益性首先需要政府加大对公共医疗卫生服务的投入,并加强对不合理逐利行为的监督与惩罚。The public welfare of public hospitals has been a controversial issue for a long time. This essay defines the public welfare in the view of public administration, and points out that the public welfare of public hospitals is the positive externality brought by medical products and service. The essay considers the main reason of the reduction of the public welfare of public hospi- tals is not the deficiency of fiscal input, but the insufficiency of financial support on public health, which mostly reflects the public welfare, and public hospitals can pursue profits reasonably and legally. In order to enhance the public welfare of public hospitals, the government has to increase the financial support on public health, and emphasizes the scrutiny and penalty of unreasonable prof- it seeking behaviors.
