摘  要:目的:探讨昆明地区冠心病(CHD)人群中CYP2C19基因多态性分布情况,为本地区冠心病人群个体化药物治疗提供依据。方法:采用Sequenom Mass ARRAY飞行质谱方法检测昆明地区根据知性同意原则收集的198例正常人(对照组)和514例冠心病患者(冠心病组)血液样本DNA中细胞色素P450(CYP)2C19基因基因多态性,计算基因型、等位基因、代谢表型分布频率,分析昆明地区冠心病人群CYP2C19基因多态分布情况。结果:CYP2C19*1,CYP2C19*2,CYP2C19*3等位基因在2组总体中的分布频率为68.94%vs.66.93,28.03%vs.27.63%,3.03%vs.5.45%,2组男性中的分布频率为68.53%vs.67.40%,28.32%vs.26.99%,3.15%vs.5.60%,2组女性中的分布频率为70.00%vs.66.00%,27.27%vs.28.86%,2.73%vs.5.14%。无论在总体还是男性群体或是女性群体中正常对照组与冠心病组等位基因分布频率差异均无统计学意义(P>0.05)。昆明地区冠心病人群等位基因分布频率与武汉地区、南京地区比较差异有统计学意义(P<0.05),与淮海地区、重庆地区、南方、陕西地区以及河南地区比较差异无统计学意义(P>0.05)。结论:昆明地区人群CYP2C19基因多态性与冠心病不相关,冠心病人群CYP2C19基因多态性分布与国内其他地区人群相似。Objective: To observe the distribution of genetic polymorphism of CYP2 C19 in patients with coronary heart disease( CHD) so as to develop personalized medicine studies in Kunming population. Methods: 198 healthy subjects( control group) and 514 patients of CHD( CHD group) living in Kunming were recruited. Two polymorphisms in CYP2 C19 gene were genotyped using the SEQUENOM Mass-ARRAY system. Results: The frequency of CYP2 C19* 1,CYP2 C19* 2,CYP2 C19* 3 in the control and CHD groups were respectively present as 68. 94% vs. 66. 93,28. 03% vs. 27. 63%,3. 03% vs. 5. 45%. In males of the two groups,the frequencies of CYP2 C19* 1,CYP2 C19* 2,CYP2 C19 * 3 were respectively present as 68. 53% vs. 67. 40%,28. 32% vs. 26. 99%,3. 15% vs.5. 60%. In females of the two groups,the frequencies of CYP2 C19 * 1,CYP2 C19 * 2,CYP2 C19 * 3 were respectively present as70. 00% vs. 66. 00%,27. 27% vs. 28. 86%,2. 73% vs. 5. 14%. There was no association between the CYP2 C19 alleles and CHD in Kunming male or female populations. The distribution of CYP2 C19 alleles of patients with CHD from Kunming area is consistent with that of Huaihai,Chongqin,Southern,Shanxi and Henan regions,except for Wuhan and Nanjing areas. Conclusion: There was no association between CYP2 C19 gene polymorphisms and CHD in Kunming population. The distribution of CYP2 C19 gene polymorphisms in CHD patients from Kunming area is consistent with that of other regions of China.
