下肢深静脉血栓中医辨证标准存在的问题与对策研究

摘　　要：通过对《中医病证诊断疗效标准》《周围血管疾病中西医诊疗学》《中药新药临床研究指导原则》《周围血管病中医研究最新全书》及统编教材《中医外科学》《中西医结合外科学》等多种权威文献,以及学术界20年来发表的文献中关于下肢深静脉血栓形成中医辨证的综合考察分析,揭示权威文献之间、学术界之间、权威文献与学术界之间下肢深静脉血栓形成中医辨证存在的各种问题,借以提出该病的中医辨证规范思路和方法。研究认为,该病的中医辨证存在如下问题:(1)标准不规范,各标准差异性太大;(2)各证所属症状繁杂,与病不相干的症状甚多;(3)同证异名、同症异名十分普遍;(4)临床辨证各行其是,中医辨证标准形同虚设。故应在统一证名、规范症状的基础上,对证和各证所属症状进行合理删减;开展下肢深静脉血栓形成前瞻性中医辨证规范化研究。This study has been based on the comprehensive analysis of academic documents in the past 20 years and so many kinds of authoritative documents,such as Curative Effect of TCM Syndrome Diagnoses Standards,Diagnosis and Treatment of Peripheral Vascular Disease with Traditional Chinese and Western Medicine,Clinical guideline of new drugs for TCM,The latest research on peripheral vascular disease in traditional Chinese Medicine and state-complied textbooks such as Surgery of Traditional Chinese Medicine and Surgery of Integrated Traditional Chinese and Western Medicine.It reveals different kinds of dialectical problem of TCM among authoritative documents,academia,and between authoritative documents and academia about deep venous thrombosis.This study presents normative ideas and methods of TCM dialectics about deep venous thrombosis.Studies have shown that the dialectical standards of TCM are nonstandard about the disease,and differences between standards are great,what's more,symptoms of the disease are complex,and there are so many symptoms that are not connected with this disease.In addition,the phenomenon is universal,that the same disease has different names and the same symptom of a disease has different names.Clinical dialectics acts in their own ways,and standards of TCM dialectics have performed practically no function.Therefore,based on unified names and normative symptoms,we should reasonably delete the uncorrelated disease and its symptoms,and at the same time,we should to carry out prospective and normative dialectical studies about deep venous thrombosis.
