纤维支气管镜肺泡灌洗术对重症肺炎患者炎性因子及肺功能的影响

摘　　要：目的探究纤维支气管镜肺泡灌洗术对重症肺炎患者炎性因子及肺功能的影响。方法选取2015年2月~2016年12月我院收治的80例重症肺炎患者作为研究对象,根据随机数字表法将所有患者分为两组,每组各40例。对照组患者实施常规治疗,观察组患者在此基础上予以纤维支气管镜肺泡灌洗术治疗。比较两组患者的炎性和肺功能指标。结果治疗前,两组患者的STREM-1、HMGB-1、CD11b阳性中性粒细胞、FVC、FEV_1及FVC/FEV_1比较,差异均无统计学意义(P>0.05)。治疗后,两组患者的STREM-1、HMGB-1、CD11b阳性中性粒细胞、FVC、FEV_1及FVC/FEV_1均明显低于本组治疗前,观察组患者的STREM-1、HMGB-1、CD11b阳性中性粒细胞均低于对照组,FVC、FEV_1及FVC/FEV_1均高于对照组,差异有统计学意义(P<0.05)。结论对重症肺炎患者实施纤维支气管镜肺泡灌洗术治疗效果显著,能有效改善患者的肺功能,减轻炎症反应。Objective To investigate the effect of bronchofibroscopic bronchoalveolar lavage on inflammatory factor and lung function in patients with severe pneumonia.Methods 80 patients with severe pneumonia treated in our hospital from February 2015 to December 2016 were selected as subjects,and they were divided into two groups by random number table method,40 cases in each group.Control group was given conventional treatment,while observation group was given bronchofibroscopic bronchoalveolar lavage based on control group.The inflammatory factors and lung function factors were compared between the two groups.Results Before treatment,the STREM ~ 1,HMGB- 1,CD 1 lb positive neutrophils,FVC,FEV1 and FVC/FEVI were compared of patients between the two groups,and the differences were not statistically significant （P〉0.05）.After treatment,the STREM-1,HMGB-1,CD1 lb positive neutrophils,FVC,FEV and FVC/ FEVl of patients in the two groups were significantly lower than before treatment,the STREM-1,HMGB-I and CDllb positive neutrophils of patients in the observation group were lower than those in the control group,while the FVC,FEVi and FVC/FEVI in the observation group were higher than those in the control group,the differences were statistically significant （P〈0.05）.Conclusion Bronchofibroscopic bronchoalveolar lavage for patients with severe pneumonia is effective,it can improve lung function and reduce inflammatory response.
