摘 要:目的探讨噻托溴铵联合无创通气对慢性阻塞性肺疾病(COPD)并Ⅱ型呼吸衰竭患者的疗效观察。方法选取2014年2月~2016年12月我院收治的108例COPD并Ⅱ型呼吸衰竭患者作为研究对象,按照随机数字表法将其分为对照组和研究组,每组各54例。对照组患者予以无创通气治疗,研究组患者予以噻托溴铵联合无创通气治疗。比较两组患者的呼吸困难症状评分、肺功能相关指标、血气相关指标以及6 min步行距离。结果干预后,研究组患者的呼吸困难症状评分显著低于对照组,差异有统计学意义(P<0.05)。研究组患者的Pa O2高于对照组,Pa CO2低于对照组,差异有统计学意义(P<0.05)。研究组患者的FEV1、FVC、FEV1/FVC均显著高于对照组,差异有统计学意义(P<0.05)。研究组患者的6 min步行距离显著长于对照组,差异有统计学意义(P<0.05)。结论噻托溴铵联合无创通气治疗COPD并Ⅱ型呼吸衰竭,可显著改善患者的肺功能及血气指标,降低呼吸困难评分,提高运动耐力,值得推广应用。Objective To investigate the effect of Tiotropium combined with non-invasive ventilation on chronic obstructive pulmonary disease and pulmonary function in patients with type Ⅱ respiratory failure.Methods 108 cases of COPD and type Ⅱ respiratory failure patients treated in our hospital from February 2014 to December 2016 were selected as subjects and were divided into the control group and the study group according to the random number method, 54 cases in each group.The patients in the control group were treated with non-invasive ventilation,and patients in the study group were treated with Tiotropium combined with non-invasive ventilation.The dyspnea symptom scores,lung function indexs,blood gas correlation indexs and 6 min walking distance were compared between the two groups.Results After the intervention,the score of dyspnea symptoms in the study group was significantly lower than those in the control group,the difference was statistically significant (P〈0.05).The PaO2 of the study group was significantly higher than that of the control group,and the PaCO2 of the study group was lower than that of the control group,the differences were statistically significant (P〈0.05).The FEVI,FVC and FEV1/FVC in the study group were significantly higher than those in the control group,the differences were statistically significant (P〈0.05).The walking distance of 6 min in the study group was significantly long than that in the control group,the difference was statistically significant (P〈0.05).Conclusion Tiotropium combined with noninvasive ventilation in the treatment of COPD patients with type Ⅱ respiratory failure can significantly improve the pulmonary function and blood gas index,reducing dyspnea score,improve exercise tolerance,and be worth promoting the application.
