大剂量环磷酰胺辅助治疗肾病综合征型紫癜性肾炎患儿的临床效果

摘　　要：目的探讨大剂量环磷酰胺联合泼尼松治疗肾病综合征型紫癜性肾炎的临床效果。方法选取我院2014年2月~2016年3月收治的60例肾病综合征型紫癜性肾炎患儿,按照随机数字表法分为对照组和研究组,每组各30例。对照组患儿采用泼尼松治疗,研究组患儿给予大剂量环磷酰胺联合泼尼松治疗。比较两组患儿的临床效果、24 h尿蛋白量、外周血白细胞(WBC)计数、尿红细胞计数、总胆固醇(TC)、三酰甘油(TG)、血肌酐(Scr)、血尿素氮(BUN)、血浆清蛋白(ABL)、肾外症状及不良反应发生情况。结果研究组患儿总缓解率为93.33%,显著高于对照组的66.66%,差异有统计学意义(P<0.05)。干预后研究组患儿24 h尿蛋白、TG、Scr、BUN、TC、尿红细胞计数水平,与对照组比较差异均有统计学意义(P<0.05)。干预后研究组出现Cushing综合征比例显著低于对照组(P<0.05),镜下血尿比例显著低于对照组,差异均有统计学意义(P<0.05)。结论大剂量环磷酰胺联合泼尼松治疗肾病综合征型紫癜性肾炎患儿疗效确切,患儿不良反应较少,可显著改善患儿肾功能。Objective To investigate the effect of high dose Cyclophosphamide combined with prednisone in children with nephrotic syndrome and purpura nephritis.Methods 60 cases of nephrotic syndrome type purpura nephritis were selected from February 2014 to March 2016.According to the random number table method,the control group and the study group were divided into 30 cases.The control group was treated with strong pine treatment,the study group to give high-dose Cyclophosphamide combined with prednisone treatment.Compared of the clinical efficacy of the two groups, 24 h urinary protein,peripheral white blood cell（WBC） count,urine red blood cell count,total cholesterol（TC）,triglyceride （TG）,serum creatinine（Scr）,hlood urea nitrogen（BUN）,plasma albumin（ABL）,extra renal symptoms and adverse reaction should the occurrence.Results The total remission rate of the study group （93.33%） was significantly higher than that of the control group （66.66%）（P〈0.05）.After intervention,the levels of 24 h urinary protein,TG,Scr,BUN,TC,and urine red blood cell count in the study group were significantly different from those in the control group （P〈0.05）.Afier the intervention,the proportion of Cushing syndrome in the study group was significantly less than that in the control group（P〈0.05）, and the proportion of microscopic hematuria was significantly smaller than that of the control group （P〈0.05）.Conclusion High dose Cyclophosphamide combined with prednisone in the treatment of children with nephrotie syndrome type purpura nephritis curative effect is exact,less adverse reactions in patients,can significantly improve renal function.
