瑞芬太尼复合丙泊酚对宫腔镜下宫腔分粘术中血流动力学的影响

摘　　要：目的观察瑞芬太尼复合丙泊酚对宫腔镜下宫腔分粘术中血流动力学的影响。方法选取我院2015年2月17日~2017年2月17日行宫腔镜下宫腔分粘术治疗的100例宫腔粘连患者,依据随机数字表法分为对照组与观察组,各50例。对照组采用瑞芬太尼麻醉,观察组采用瑞芬太尼复合丙泊酚麻醉。比较两组患者的血液动力学指标及唤醒时间、拔管时间。结果对照组拔管时的MAP水平高于诱导前,差异有统计学意义(P<0.05)。观察组手术开始20 min、拔管前、拔管后5 min的HR、MAP水平低于诱导前,差异有统计学意义(P<0.05)。两组诱导前的HR、MAP水平比较,差异无统计学意义(P>0.05)。观察组手术开始20 min、拔管前、拔管后5 min的HR、MAP水平低于对照组,差异有统计学意义(P<0.05)。观察组的术后唤醒时间、拔管时间短于对照组,差异有统计学意义(P<0.05)。结论瑞芬太尼复合丙泊酚对宫腔镜下宫腔分粘术血流动力学的影响较弱,麻醉效果良好,具有较好的临床推广价值。Objective To observe the influence of Remifentanil and Propofol on hemodynamics in transcervical resection of adhesions.Methods 100 cases of intrauterine adhesions underwent transcervical resection of adhesions from February 17,2015 to February 17,2017 in our hospital were selected and divided into the control group and the observation group according to the random number table methods,50 cases in each group.The control group was given with Remifentanil, and the observation group was given Rremifentanil combined with Propofol anesthesia.The index of bemodynamics,the wake-up time,as well as the extubation time of the two groups were compared.Results The MAP level in extubation in the control group was higher than that before the induction,with significant difference （P〈0.05）.The level of HR and MAP of the observation group at 20 min of operation begins,before extubation,at 5 rain after extubation was lower than that before the operation,with significant difference （P〈0.05）.There was no significant difference in the level of HR and MAP between the two groups before induction （P〉0.05）.The level of HR and MAP of the observation group at 20 min of operation begins,before extubation,at 5 rain .after extubation was lower than that of the control group,with significant difference （P〈0.05）.The wake-up time and the extubation time in the observation group were shorter than those in the control group,with significant difference （P〈0.05）.Conelusion Application of Remifentanil combined with Propofol has a lower influence of hemodynamics in＇transcervical resection of adhesions,the effect of anesthesia is good,and it has good clinical promotion value.
