摘 要:目的探讨正常孕妇尿沉渣各项指标是否有异于正常未孕女性,并确定其参考范围。方法随机选取2017年6月至8月在北京大学人民医院进行孕期保健的孕妇624例为试验组,同期在本院体检中心查体的健康非妊娠女性576例为对照组。剔除其中的异常人群和异常数据。采用UF-1000i流式自动细胞分析仪(SYSMEX)检测两组尿沉渣中白细胞(WBC)、红细胞(RBC)、细菌(BACT)、上皮细胞(EC)、小圆上皮细胞(SRC)和黏液丝(MUS)的浓度,将两组间指标进行比较;进一步将试验组分为孕早期(〈12周)、孕中期(12~27+6周)和孕晚期(≥28周)三组进行比较。结果最终试验组纳入505例孕期健康女性,对照组纳入466例非孕期健康女性。试验组中早孕期141例,中孕期176例,晚孕期188例。孕妇尿沉渣中WBC、RBC、BACT、EC、SRC、MUS的参考区间分别为0~29.4/μl、0~22.0/μl、0~695.9/μl、0~28.0/μl、0~7.6/μl、0~2.8/μl。和非孕期妇女相比,各项检测指标浓度均有明显变化,WBC、BACT、EC、SRC的浓度明显升高(P〈0.01);但RBC、MUS的浓度却明显降低(P〈0.01)。WBC、BACT、EC、SRC的浓度在不同孕期无明显变化(P〉0.05)。但RBC的浓度随着孕周的增加而下降,差异有统计学意义(P〈0.05)。和早孕期相比,中孕期和晚孕期MUS的浓度明显下降(P〈0.01),但后两者间差异无统计学意义(P〉0.05)。结论和非孕期妇女相比,妊娠期女性尿沉渣各项指标的浓度确实都有不同,各实验室应建立妊娠期专门的尿沉渣检查指标参考值范围。Objective To establish reference intervals of white blood cell(WBC),red blood cell(RBC),bacteria(BACT),squamous epithelial cell(EC),small round epithelial cell(SRC) and mucus(MUS) for urine sediment in pregnant women,to provide the differences between pregnancy with non-pregnancy and the differences among gestational trimesters.Methods A prospective analysis was conducted in Peking university People's hospital from June to August 2017,including pregnancy group(505 healthy pregnant women) and control group(466 healthy non-pregnant women).All the women had no history of UTI or any other disease.The urine specimens were analyzed by the Sysmex UF1000 i flow cytometry analysis followed by a manual correction.Results The pregnancy reference intervals of WBC,RBC,BACT,EC,SRC,and MUS were 0~29.4/μl,0~22.0/μl,0~695.9/μl,0~ 28.0/μl,0~ 7.6/μl and 0~2.8/μl,respectively.The concentration of WBC,BACT,EC,and SRC,were significantly higher in the pregnancy group compared to the control group(P 〈0.01),while the concentration of RBC and MUS were significantly lower in the pregnancy group(P〈 0.01).The comparison of trimesters in WBC,BACT,EC,and SRC were statistically indistinguishable(P 〉0.05).The concentration of RBC decreased significantly with the increase of gestational trimester.Conclusion There were significant differences of each parameter between healthy pregnant women and non-pregnant women,but no difference among trimesters of pregnancy.
