中心静脉置管在肿瘤内科临床治疗中的应用价值

摘　　要：目的:探讨中心静脉置管在肿瘤内科的用途及其价值。方法:收治采用两种静脉置管的患者102例,其中采用经锁骨下中心静脉置管(CVC)84例,经外周静脉穿刺中心静脉置管(PICC)18例。从成功率、感染率、舒适度等方面对这两种置管方式进行评价。结果:102例患者共出现感染6例,发生率2.94%;从机械性并发症发生情况来说,(1)经锁骨下CVC:患者84例发生的并发症依次为导管滑脱1例(1.23%),误刺锁骨下动脉1例(1.23%),导管异位(误入颈内或颈总或颈前静脉)2例(2.38%),同侧颈肩部或耳后酸胀不适或伴疼痛3例(3.57%);(2)PICC:患者18例发生的并发症依次为局部疼痛2例(11.11%),局部肿胀1例(5.56%),导管外移和导管阻塞各1例(5.56%)。结论:在肿瘤内科临床治疗中应用中心静脉置管,可较好地降低患者并发症的发生率,清除各类风险隐患,提高患者满意度,有利于患者预后,应用价值较高。Objective:To investigate the use of central venous catheter in oncology and its value.Methods:102 patients with two kinds of venous catheterization were selected.Among them,84 cases were treated with subclavian central venous catheter(CVC),and 18 cases with peripherally inserted central venous catheter(PICC).We evaluated these two catheter methods from the success rate,infection rate,comfort and other aspects.Results:There were 6 cases of infection in 102 cases,and the incidence rate was 2.94%.(1)subclavian CVC:the complications of 84 cases were catheter slippage in 1 case(1.23%);false subclavian artery in 1 case(1.23%);2 cases(2.38%)had ectopic catheter(internal or jugular or jugular vein);there were 3 cases(3.57%)with soreness or pain in the neck,shoulder or back of the same side.(2)PICC:the complications of 18 cases were local pain in 2 cases(11.11%);local swelling in 1 case(5.56%);catheter moving and catheter occlusion of 1 case(5.56%).Conclusion:Central venous catheterization can reduce the incidence of complications in the clinical treatment of tumor,clear all kinds of potential risks,improve patients' satisfaction,improve prognosis,with higher application value.
