BIPAP对慢性阻塞性肺气肿合并呼吸衰竭的诊疗效果观察

摘　　要：目的:探讨双水平气道正压通气(Bi-level positive airway pressure,BIPAP)对慢性阻塞性肺气肿合并呼吸衰竭患者的诊疗效果。方法:随机抽取慢性阻塞性肺气肿合并呼吸衰竭患者90例,按所采取的不同治疗方法分为观察组(共48例)和对照组(共42例)。所有患者均给予常规治疗,对照组加用呼吸兴奋剂治疗,观察组给予BIPAP呼吸机治疗。比较两组治疗前后心率(HR)、呼吸频率(RR)、血气分析值和生活质量改善情况。结果:治疗12 h后两组患者HR、RR、动脉氧分压(Pa O2)、二氧化碳分压(Pa CO2)均较治疗前明显改善,差异有统计学意义(P<0.05),观察组改善程度优于对照组,差异有统计学意义(P<0.05)。治疗12周后两组圣乔治呼吸问卷(SGRQ)各维度评分均较治疗前明显下降,差异有统计学意义(P<0.05),观察组各维度评分均低于对照组,差异有统计学意义(P<0.05)。结论:BIPAP可显著缓解慢性阻塞性肺气肿合并呼吸衰竭患者的临床症状,改善高生活质量。Objective To investigate the effect of bi-level positive airway pressure（BIPAP） in patients with chronic obstructive pulmonary emphysema combined with respiratory failure. Method Ninety patients with chronic obstructive pulmonary emphysema combined with respiratory failure diagnosed by respiratory medicine were randomly selected and divided into observation group（48 cases） and control group（42 cases） according to different treatment methods. All patients were given routine treatment,the control group with respiratory stimulant therapy,the observation group given BIPAP ventilator treatment. The changes of heart rate（HR）,respiratory rate（RR）,blood gas analysis and quality of life were compared before and after treatment. Results After treatment for 12 hours,HR,RR,arterial oxygen partial pressure（Pa O2） and partial pressure of carbon dioxide（Pa CO2） were significantly improved（P〈0. 05）,and the improvement was better in the observation group than in the control group（P〈0. 05）. The scores of the two groups were significantly lower than those of the control group（P〈0. 05）. The scores of the two groups were significantly lower than those of the control group（P〈0. 05）. Conclusion BIPAP can significantly relieve the clinical symptoms of patients with chronic obstructive pulmonary emphysema combined with respiratory failure and improve the quality of life.
