盐酸戊乙奎醚与阿托品用于急性有机磷农药中毒过程中谵妄的对比观察

摘　　要：目的:观察比较长托宁(盐酸戊乙奎醚)与阿托品治疗急性有机磷农药中毒(acute organophosphorus pesticide poisoning,AOPP)过程中的谵妄症状。方法:选取2012年1月-2016年12月笔者所在医院收治的56例AOPP患者,将其按治疗方法分为长托宁组和阿托品组,各28例。阿托品组选用硫酸阿托品注射液治疗,长托宁组选用盐酸戊乙奎醚注射液治疗。观察并记录患者用药后出现阿托品化(或长托宁化)的时间、谵妄发生率、CHE的恢复时间。结果:阿托品组中、重度中毒患者达到目标效应时间均短于长托宁组,比较差异均有统计学意义(P<0.05);长托宁组患者谵妄发生率高于阿托品组,但差异无统计学意义(P>0.05);长托宁组患者CHE活性恢复时间为(6.82±3.68)d,短于阿托品组的(10.42±4.58)d,比较差异有统计学意义(P<0.05)。结论:临床选择救治有机磷农药中毒的药物时,需要综合考虑,同时把握好用药剂量,争取在达到最佳救治效果的同时,把副反应降到最低。Objective： To compare the delirium between Penehyclidine Hydrochloride and Atropine in treating acute organophosphorus pesticide poisoning（AOPP）.Method： From January 2012 to December 2016, 56 cases of patients with AOPP admitted in the author＇s hospital were selected and randomly divided into the Penehyclidine Hydrochloride group and the Atropine group, each with 28 cases.The Atropine group was treated with Atropine sulphate injection and the Penehyclidine Hydrochloride group was treated with Penehyclidine Hydrochloride injection.The time of reaching the best effect of Atropinization（or Penehyelidine Hydrochloride）, the incidence of delirium after the injection and recovery time of CHR were monitored and recorded.Result： The time of moderate and severe poisoning patients to achieve the target effect in the Atropine group were shorter than the Penehyclidine Hydrochloride group, the differences were statistically significant（P〈0.05）.The incidence of delirium in the Penehyclidine Hydrochloride group was higher than that in the Atropine group, but the difference was not statistically significant（P〉0.05）.The recovery time of CHR was （6.82 ±3.68）d in the Penehyclidine Hydrochloride group and shorter than （10.42 ±4.58）d in the Atropine group, the difference was statistically signifieant（P〈0.05）.Conclusion： Clinical options for the treatment of organophosphorus pesticide poisoning drugs need to be considered, and take a good dosage, strive to achieve the best treatment effect, while the side effects to a minimum.
